REGISTERED NURSE CARE MANAGER (RN) needed for Cherokee County, GA. This is a full time, salaried position that has uncapped earnings and offers a $15000 sign on bonus along with many other benefits. Home Health experience is preferred but not required! We gladly and properly train a RN with the desire to work in the home health setting.

Responsibilities
Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
Complete all documentation and records in a timely and accurate manner
Follow established policies and procedures as related to the scope of practice
Maintain continuity of patient care in a home care setting
LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

• Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
