Remote Social Worker (Lmsw) - Texas Child Health Access Through Telemedicine - Intake Assessor

Remote Social Worker (LMSW) to work as an Intake Assessor with our Texas Child Health Access Through Telemedicine program. The TCHATT Program is designed to deliver free telemedicine services for students to assist with identifying or assessing mental health needs and providing access to mental health care services.

Location: Position is currently a remote role but employee may be required to come into the office periodically for trainings, meetings, etc. Must be located in the Greater Houston/commutable area in order to work with local patient population and schools.

Position Summary:

Independently provides formal assessment of patients and their guardians on patient’s presenting mental health concerns.

Position Key Accountabilities:

1. Meets with TCHATT patients and their guardians and asks a series of questions concerning history and current functioning of mental health

2. Plans course of treatment or refers cases to appropriate professionals, institutions or agencies to ensure that patients receive appropriate treatment or referral if patient not appropriate for TCHATT
3. Obtains data from patients and/or relatives in the best interest of their problems to assist in the development of a number of significant teaching endeavors, programs, and social services.
4. With the objective of meeting total patient care, assesses patients and/or guardian to gain understanding of the individual's problems and needs. Collaborates with the psychiatrist and other members of the multidisciplinary team to coordinate care, enhance the patient experience and achieve collaborative goals.
5. Conducts initial and ongoing assessments of TCHATT patients to determine mental health needs and establish appropriate levels of care
6. Maintains all documentation in multiple data management systems in a timely manner.

7. Provides education and presentations to community providers and other healthcare providers on mental health and the impact of psychosocial factors on care need.
8. Utilizes crisis intervention, conflict resolution, and mediation to address patient/family concerns.
9. Responds to emergency situations.
10. Performs other duties as assigned.

Certification/Skills:

Licensure as a Master Social Worker (LMSW) by the Texas Board of Social Work.

Minimum Education:

Masters Degree in Social Work or related field.

Minimum Experience:

One year of social work experience required. Medical social work experience is preferred.

Physical Requirements:

Exerts up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
