Job Description
Job Purpose: Promotes and restores clients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing support to clients, and families; supervising assigned team members. Summary: This position is primarily responsible for providing state assessments per all state regulations, initial and ongoing assessments of each client, and supervision of Unlicensed Personnel (PCW). This position is responsible for the overall direction, coordination, and evaluation of the PCW in accordance with the medical assistance personal care program, in the States of Wisconsin. The RN may delegate nursing tasks to a trained and competent individual that are within the RN’s scope of practice. The RN carries out supervisory responsibility in accordance with the company’s policies and applicable law. Essential Duties and Responsibilities: Abide by all federal and state regulations pertaining to the agency’s services. Adheres to Chapter DHS 105.17 (2); Qualifications and Duties of a Registered Nurse. Ability to comprehend and implement the agency’s policies and procedures. Perform initial and renewal assessments of the health and fitness of clients. Identifies client care needs by establishing personal rapport with potential and actual clients and other persons able to understand care needs. Recognizes the strengths and the ability of individuals to express preferences and to make choices. Complete check of the health of the client through observation, client history, and symptoms. Receive written physician’s orders, to be renewed once every three months or up to one year if physician deems it appropriate or when the client’s needs change. Notify physician, medical personnel, or any other representative of significant changes observed or reported in the client’s condition and or environment. Promotes client's independence by; developing a care plan with consideration to client’s choice, teaching staff, client, friends, and family to understand condition, medications (if applicable), and self-care skills; answering questions. Give written instructions with a demonstration of care/ongoing evaluation of staff, and implement a reporting process. Assures quality of care by adhering to therapeutic standards; making or recommending necessary adjustments; and following standards of care set by the state board of nursing, state Nurse Practice Act, and other governing agency regulations. Resolves client problems and needs by utilizing multidisciplinary team strategies. Maintains a safe and clean working environment by complying with the agency’s procedures, rules, and regulations; calling for assistance from health care support personnel. Protects clients and employees by adhering to infection-control policies and protocols. Completion of Personal Care Screening Tool, Nursing Assessment, Plan of Care (485), Client Specific, and any other necessary documentation. Documents client care services by charting in client and agency records. Maintains client confidence and protects operations by keeping information confidential. Maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; and participating in team problem-solving methods. Contributes to team effort by accomplishing related results as needed. All other duties are assigned by the owner.
Supervisory Responsibilities: This position supervises ULPs assigned to work with all clients who receive Medical Assistance.
This position is responsible for the overall direction, coordination, and evaluation of the ULPs.
The RN/MA Manager carries out supervisory responsibility in accordance with the company’s policies and applicable law. Skills/Qualifications: Clinical Skills, Nursing Skills, Bedside Manner, Infection Control. Medical Teamwork. Multi-tasking, Listening, Verbal Communication, computer skills, working knowledge of modern business software. Health Promotion and Maintenance. Current Wisconsin Registered Nurse License. One year of supervisory or administrative experience pertaining to home care, personal care, and related services. Training and other Required Duties: Receive appropriate training and adhere to job performance expectations and standards for all aspects of duties. Complete all mandatory new hire requirements. Adhere to Peaceful Living, LLC policies and procedures outlined in the policy manual, Employee Handbook. Follow all safety guidelines.
