DEPARTMENT: Home Health and Hospice
REPORTS TO: Home Health and Hospice Director
JOB SUMMARY
The Home Health and Hospice RN provides skilled nursing assessment, planning and care to maximize the comfort and health of patients and families. The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures.
Position requires travel around a 50-mile radius of Cody; reliable transportation necessary, valid driver's license and vehicular insurance. Must be available for rotating work schedule.
JOB REQUIREMENTS
Current Wyoming Register Nurse License
EXPERIENCE
Home Care experience preferred: 2 years nursing experience required.
SKILLS
Ability to work autonomously; excellent assessment and communication skills.
REQUIRED CERTIFICATIONS
BLS and a current driver’s license
ESSENTIAL FUNCTIONS
- Assist nursing staff in other departments as requested and qualified when need arises.
- Prepare nurses notes.
- Regularly reevaluate the nursing needs of patients and prepares recertification.
- Complete 24 hours of in-service training per year.
- Perform supervisory visits of HHA.
- Share in providing 24-hour, seven-day a week coverage to Hospice patients/families.
- Comply with Medicare conditions of participation.
- Perform 4-5 nursing visits/day.
- Nursing assessments and patient evaluations.
- Initiates the patient's plan of treatment and any necessary revisions.
- Provide appropriate support at time of death.
- Provide total patient care for a team of patients.
- Inform MD and nursing supervisor of changes in patients' condition and needs.
SECONDARY FUNCTIONS
- Complete D/C summaries on primary patients.
- Coordinate treatment with paramedical personnel.
- Schedules home visits as assigned.
- Participates in in-service programs, D/C planning, case conferences, IDT meetings and staff meetings.
- Alternate on call hours with other RN's.
