Description:

As a Registered Nurse (RN) at Fenway Health you will provide compassionate, evidence-based, and efficient care to individuals, families, communities, and patient populations. The Registered Nurse’s care delivery is consistent with the Massachusetts Board of Registration in Nursing, the ANA Scope and Standards of Practice, and the ANA Code of Ethics. The RN demonstrates the professional role obligations of. Professional accountability enriches the RN's engagement as a leader in promoting an inter-professional culture of collaborative decision-making, innovation, life-long learning, and teamwork.

At Fenway Health the RN will provide quality nursing care to patients consistent with the care philosophy of patient-centeredness, non-judgmental, harm reduction, behavioral health integration, and team-based care. The nurse serves in a clinical leadership role to the care team with primary responsibilities for disease management, care coordination, and advice/triage needs for the care team's panel of patients. The nurse ensures that the team functions smoothly and efficiently while keeping the patients' well-being paramount. The RN will set a positive example of nursing for all Fenway Health staff, residents, and visiting learners; and will always conduct their self in a respectful, courteous, and professional manner consistent with the Fenway Health Mission “to enhance the wellbeing of the LGBTQIA+ community and all people in our neighborhoods and beyond through access to the highest quality health care, education, research, and advocacy.” The schedule is based on clinic needs and may include covering evening and Saturday clinics on a rotating basis.

Principal Duties and Responsibilities

  • Participates in planning, implementing, and monitoring nursing goals and objectives consistent with the Fenway Health Strategic Plan.
  • Identifies complexities within the healthcare system, and Fenway Health, and participates in resolving barriers to effective, efficient, and fiscally responsible care delivery.
  • Exhibits a positive attitude with patients and establishes professional and respectful relationships with the internal and external healthcare team members, agencies, and healthcare facilities.
  • Adheres to organizational and departmental policies and procedures in collaboration with other departments and in accordance with state, federal, and organizational guidelines.
  • Demonstrates initiative and commitment to continuously improve services and processes that positively impact quality and utilization of care, patient experience, and staff job satisfaction.
  • Implements direct and indirect nursing care consistent with evidence-based practices, hospital policies and procedures, scope and standards of practice, and Nursing’s Code of Ethics.

  • Assures a sensitivity to recognize, appreciate, and incorporate individual differences and preferences in the provision of culturally competent care for the patient and with the care team.
  • Speaks up immediately about concerns regarding assigned responsibilities and available resources.
  • Attends meetings and participates in committee functions and activities as nominated or assigned.

  • Evaluates own knowledge, as well as nursing peers, and nursing practice in relation to professional practice standards and evidence-based knowledge in consultation with peers and colleagues.
  • Provides competent, compassionate nursing care based on the nursing process, clinical reasoning, objective scientific principles, and the patient’s subjective experiences.
  • Documents patient information accurately and in a timely manner in the EHR.
  • Influences appropriate utilization of healthcare resources by helping to coordinate patient care, encouraging patient involvement in disease management and wellness programs, and conducting follow-up care for acute care or psychiatric admissions, emergency department visits, specialist visits, and sub-acute / rehabilitation care stays.
  • Demonstrates initiative and commitment to continuously improve services and processes that positively impact patient care and organizational goals.
  • Conducts virtual, telephonic, or in-person nurse assessment and triage to assess the severity of the patient’s health concerns using approved protocols and resources to advise appropriately.

Knowledge and Abilities

  • Excellent written, verbal and listening community abilities. Communicate appropriately and clearly to staff and providers.
  • Ability to make independent decisions in accordance with established policies and procedures. Decisions and problem solving require a combination of analysis, evaluation and interpretive thinking.
  • Computer literacy, including by not limited to, data entry, retrieval and report generation.
  • Knowledge of and appreciation for cultural diversity and low literacy issues in care provision.
  • Demonstrates ability to compile patient data and prepare outcome analysis.
  • Ability and willingness to self-motivate, to prioritize and change processes to improve effectiveness and efficiency. Adapts to changing patient or organizational priorities.
  • Ability to work with patients/families of all ages and in a variety of settings, including office, facility and patients’ homes, presenting diverse physical conditions and social/cultural environments.

Requirements:
  • Proficient in the use of the computer and associated software, i.e., MS products

  • Current CPR certification required
  • Education: Associate degree in nursing

  • Current Massachusetts Registered Nurse (RN) license degree required.

Preferred Qualification

  • Board Certification (ACRN, AMB-BC, etc)
  • Two (2) years of experience in ambulatory, hospital, or public health nursing
  • Experience with working within a complex healthcare system with multiple product lines
  • Bachelor’s degree level or higher in nursing
