This is a professional Registered Nurse 3 nursing position assigned to the Lafayette Parish Health Unit in the Acadian Region 4 of the Office of Public Health and functioning as the lead worker over professional and paraprofessionals with full time programmatic responsibility in the coordination and monitoring of the Children’s Special Health Services Program (CSHS). The Lafayette (Acadian) Region Children’s Special Health Services is one of eight CSHS clinics located statewide to provide community based rehabilitative services to children from birth to age 21 who meet medical and financial guidelines. It serves a seven parish area This is a professional Registered Nurse 3 nursing position assigned to the Lafayette Parish Health Unit in the Acadian Region 4 of the Office of Public Health and functioning as the lead worker over professional and paraprofessionals with full time programmatic responsibility in the coordination and monitoring of the Children’s Special Health Services Program (CSHS). It serves a seven parish area with 3 specialty clinics. This site also sees many children who live in other regions of the state. It assists with the Title V Regional Genetics Clinic held monthly. Responsibilities also include performing generalized nursing duties according to OPH policy and procedure manual in the following programs: Reproductive Health, Genetics, Immunizations, Special Supplemental Food Program for Women, Infants, and Children (WIC), Tuberculosis, etc. Additional functions of this position include responding to emergencies and public health threats on a 24 hour basis, i.e., hurricanes, flooding, West Nile Virus, Pandemic Flu, food borne illnesses, and other public health threats as they emerge. Performance of duties may require visiting homes in dangerous or high-risk areas, responding to emergencies and public health threats on a 24-hour basis. Practice is within the scope of practice designated by the Louisiana Nurse Practice Act (LRS 37:911), adheres to the policies, procedures, and standards of the Louisiana Department of Health and Hospitals/Office of Public Health and is accountable for promoting a positive image and works harmoniously with all members of the health care team. Supervision is received from the Registered Nurse Supervisor A.

85% Serves as Lead Nurse for the Children’s Special Health Services Program housed at the Lafayette Parish Health Unit. Reviews the charts of all patients scheduled to assigned clinics to verify the physicians recommendations have been followed such as lab work and X-rays; that return appointments have been scheduled, that lab reports and X-rays are available for the physician’s review during clinic. Evaluates treatment plans conferring with other members of the treatment team such as the physician, medical social worker, nutritionist etc. by phone and/or in person when necessary. Responsible for conducting application process for Lafayette Parish clients. Reviews charts after each assigned clinic; performs follow-up activities within a time frame appropriate to the required action. Arranges with hospitals, laboratories, private clinics, etc. for hospitalizations, outpatient testing, laboratory testing, occupational or physical therapy for patients and confirms the schedules with the patients and primary care givers. Provides missed appointment follow up for those who do not attend the clinic appointment. In the absence of the clinic physician, analyzes patient’s medical problems in person or by phone to determine appropriate care and/or necessary referral. Informs school nurses, allied school personnel and CSHS personnel so they can provide the support system required for each individual patient; consults with local health unit nurses to problem solve unusual case management problems or to request specific follow-up. Assists the physicians with examinations and specialized medical procedures such as applying or removing a cast, removing sutures, etc. Confers with the patients and primary care givers after the physician’s examination to review recommendations made by the physician, to instruct them on the nature of a diagnosis or prognosis and to explain ordered tests and surgery recommendations so they can make well informed decisions for the child; dispenses prescriptions ordered by the physician and gives instructions regarding dosage, proper application of medications, possible side effects and/or allergic reactions, proper care and use of orthotics, prosthetics or casts, the procedure for catheterization and guidelines concerning surgical procedures. Educates patients in special procedures, provides preventive health education and counsels clients and families on such topics as infant care, hygiene, nutrition, parenting, safety, immunization schedules, comfort measures after immunizations, etc. Refers to other OPH services such as Early Steps, Genetics, Nurse Family Partnership etc., and other community providers and agencies for treatment, follow-up or assistance. Conducts nursing assessments on patients during clinics which include recording the medical history, checking and recording vital signs (blood pressure, heart rate, Height, weight, head circumference) which requires lifting and moving patients who are incapable of moving themselves. Collects and labels specimens according to proper laboratory procedures for laboratory analysis including newborn screening blood specimens, blood for genetic evaluation, and urine samples for dipstick analysis. Performs sample analysis on urine and blood assuring that all controls are within acceptable limits. Conforms to OSHA/CLIA regulations at all times. Uses appropriate technology to access web based programs such as LINKS for immunizations, LIMS for lab work, COMPASS for billing and various on- line required educational offerings. Conducts audits of these programs to complete the Continuous Quality Improvement Plan and assure proper billing of services.

15% Utilizes interpersonal communication skills in a customer-focused environment, which includes all disciplines of employees, visitors, and clients. Serves as an advocate for children with special needs by consultations with private agencies and school personnel concerning services for them. Provides professional nursing care, leadership, and/or surveillance in response to emergency situations in the community and statewide in the staffing of Special Needs Shelters, smallpox vaccination, Strategic National Stockpile, and other public health responses to weapons of mass destruction. Participates in OPH training for Special Needs Shelter. Maintains current CPR certification. Attends seminars, workshops, and in-service training, and other continuing education programs as required for job performance and to maintain professional expertise and licensing. Accepts and performs additional duties not listed but necessary for the smooth operation of the work unit. with 3 specialty clinics. Qualifications

MINIMUM QUALIFICATIONS:
Possession of a valid Louisiana license or temporary permit to practice professional nursing or a multi-state license issued by a Nursing Licensure Compact (NLC/eNLC) state plus two years of professional nursing experience.

SUBSTITUTIONS:
A baccalaureate degree in nursing will substitute for one year of the required experience.
A master's degree in nursing or public health will substitute for one year of the required experience.
Specialized degrees less than a doctorate will substitute for a maximum of one year of the required experience.
A doctorate in nursing or public health will substitute for the two years of the required experience.

Performs case management activities utilizing a nursing process of assessment, planning, intervention, and evaluation with assigned families. Schedules and conducts appropriate home visits and refers to community health-care organizations that focus on parental, child health and life development. Uses a Management Information System to manage data, to track family characteristics and needs services provided, and the progress toward program objectives.
Provides health education to individuals/groups regarding health risks and strategies for promoting/protecting their own health, self-care and nutrition.
Obtains Pap Smears, blood samples, and other specimens for analysis.
Interprets laboratory results for appropriate intervention and follow-up according to established Program, OSHA and CLIA protocol.
Administers immunizations and other medications; enters data of vaccines administered into LINKS computerized system. Participates on Immunization Response Teams.
Provides professional nursing care and leadership in response to emergency situations in the community and statewide on a 24-hour basis. Ensures emergency care equipment is available and in working order. Maintains CPR certification.
Participates and collaborates with Disease Intervention Specialists in the contact investigation, treatment and education of individuals infected with communicable diseases.
Evaluates the effectiveness, accessibility and quality of personal and population-based services by the collection and maintenance of data.
Responds to emergencies and public health/bioterrorist threats on a 24-hour basis.
