Summary

Provides one to one direct care to individuals with mental health disabilities who resided for a period of time in nursing homes, but who have moved to their own apartments. Scheduled for a set number of hours per week per individual according to authorization received by DMHAS CSC. Scheduled on an as needed basis yet works consistent hours each week once authorization is in place. Implements behavior and individual skill building programs and other habilitation programs. Facilitates and promotes community integration and safety.

Essential Duties And Responsibilities

• Maintains a flexible schedule to work various shifts according to the individual's plans and needs.- Follows the recovery plan and offers suggestions to the CSP when need.
