SUMMARY: This position is responsible for implementing therapy treatment plans as initiated by the PT supervising under the direction of the patient’s physician, for Community Health Network, LTD patients designed to preserve and improve abilities of patients who have conditions that are mentally, physically, developmentally, or emotionally disabling by assisting individuals to develop, recover or maintain daily living and work skills. This position is for Wayne, Stark, or both counties, your choice!

ESSENTIAL DUTIES AND RESPONSIBILITIES: This position is directly supervised by the Administrator and/ or Therapy Director within Community Health Network, LTD.

1. Complete PTA treatments per plan of care.

2. Consult with HHA team to select activity programs and coordinate occupational therapy with supervising OT.

3. Provide education to staff, caregivers, and family as necessary

4. Communication with supervising PT and giving updates on patient progress or lag

5. Travel to assigned service locations as needed to provide treatment per plan of care

6. Input documentation into the organization’s software data base for billing, completed within 24 hours.

7. Attend scheduled and/or make arrangements with the Administrator to attend required organizational and accreditation required training, or completing materials related to the material.

8. Other duties as assigned

EDUCATION AND TRAINING AND/OR EXPERIENCE:

• Ability to effectively communicate with all levels of management, staff, and individuals served.
• Must meet driver insurability requirements
• Satisfactory references and BCI background check requirements
• Professional work experience in related setting
• Current licensure in good standing with the PTAT licensing board
