Careage Home Health - King County is seeking a Home Health Physical Therapist to join our team. A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
Summary of Responsibilities
- Consult with physicians regarding change in treatment.
- Appropriately utilizes ICD-10 codes.
- Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
- Travel within King County to provide care on an intermittent basis in patients' homes.
- Take initial patient history and make initial evaluation.
- Work in conjunction with all other home care personnel in patient care planning.
- Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
- Participates in Quality Assessment and Performance Improvement activities as assigned.
- Instruct patients and family in home programs and activities of daily living.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
- Valid WA State driver's license.
- Two (2) years experience as a physical therapist, preferred.
- Home Health experience, is preferred.
- Critical thinker capable of working collaboratively with a team.
- Currently licensed as a physical therapist in the state of Washington.
- Detail-oriented, self-motivated and capable of efficient time management.
- Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
