Position Summary
The Provider Relationship Manager is responsible for developing and maintaining long-term relationships with physicians and physician office staff that are engaged in HealthMap’s Kidney Management program.

Responsibilities
  • Monitor and manage key deliverables of the physician office implementation and onboarding processes
  • Develop new provider/partner business relationships that serve as means to better member/patient engagement
  • Ensure that all SLAs and department KPIs are met and/or exceeded
  • Perform other related duties as assigned
  • Ensure Healthmap policies and procedures are followed and complies with HIPAA privacy laws and all other federal, state, and local regulations
  • Manage practice caseload
  • Act as a liaison between Healthmap and practices to ensure positive engagement and in turn performance with our program(s)
  • Work collaboratively with Clinical Practice Liaisons to establish all critical workflow processes for each assigned practice
  • Build strong cross-functional internal relationships, physician partners, and practice staff including monitoring provider performance
  • Ensure timely and successful delivery of reports to internal and external stakeholders
  • Maintain and ensure accurately and timely documentation in Compass and/or organizational systems
  • Follow all company-defined security and operational policies and procedures
Requirements
  • 3 years of pharmacy or healthcare customer service or other clinical outreach support is preferred
  • Bachelor’s degree required or 6 years of equivalent work experience
  • 3 years of experience in healthcare, managed care, physician office or account management is strongly preferred
  • Bilingual English/Spanish
  • Same state residency preferred but not required
  • Work from home within commutable range of region, city, or state
  • On Island Residency
Skills
  • Metrics-driven ability to understand key goals from the program and understand reporting results to communicate areas of opportunity to physicians and/or office staff for behavior modifications
  • Strong strategic thinking, critical thinking and analytical skills
  • Must be proficient in Microsoft Office: Outlook, Word, Excel, PowerPoint
  • Excellent presentation skills, interpersonal skills as well as excellent written and verbal communication skills to the target audience
  • Strong relationship-building skills, multitasking, prioritization, and solutions-oriented
