Trinity Health At Home, also known as Trinity Home Health Services, is a member of Trinity Health, one of the largest Catholic health care systems in the United States. Providing home care in 11 states, our services span coast to coast with 15 home health and hospice agencies.
Trinity Health At Home is recognized as a provider of choice for home health and hospice care. Trinity Health At Home network of agencies provides services seven days a week with on-call availability; assigning consistent, highly trained caregivers; offering specialized programs such as palliative care, fall prevention and advanced wound care; and meeting or exceeding all home care outcome standards . Purpose
This position will be based out of our system office in Livonia, MI. Candidate must be willing to either relocate to the Livonia area, or have a residence established to be present consistently at the system office, as the role will require sporadic travel. This is not a remote position. We do offer a full relocation benefit.
The CEO for Trinity Health at Home is accountable for the overall operations, quality and financial performance of Trinity Home Health Services (THAH). Provides leadership and direction in strategic planning, growth and operations of home and hospice programs within THAH. Ensures that programs and services are delivered in a manner consistent with the strategy and goals of Trinity Health. Facilitates collaboration among home care and hospice ministries throughout Trinity and promotes home based services' quality and efficiencies. Drives innovation and growth and optimizes alignment and relationships with other Trinity Health ministries and health segments to promote the continuum of care ensure we are serving present and future home and hospice care needs for the communities we serve. Ensuring alignment and execution of enterprise level strategy to address internal or external business & regulatory issues. Responsible for consistent operating performance and achieving financial goals. Maintaining awareness of new industry developments and standards. Provides decision support, operations &/or optimization leadership focus.

Leadership
  • Communicates vision/direction from Trinity Health and advocates for THAH needs with Trinity Health leadership
Provides executive leadership for THAH regarding the development and implementation of long-term goals and objectives which support overall strategic direction.
Provides leadership and guidance to leadership teams, fostering collaborative planning, innovation and decision-making.​ Drives Senior Leadership Team Development​.
Seeks out, embraces and capitalizes on the advantages of regional and system resources, diversity, scale, replication opportunities, people and intellectual capital. ​
Direction and Growth
Provides executive leadership and directly supervises THAH Leadership ensuring the achievement of financial, operational and strategic plans.
Ensures appropriate Human Resource programs are supporting and sustaining the growth of THAH.
Responsible for the overall growth of THAH and collaboration with Trinity Health.
Models innovation and growth in his/her leadership. Works with Regional Health Ministries (RHM's) to promote integration of the continuum of care for our communities.
Champions change, innovation and improvement by seeking out and employing collaborative and effective change management and performance improvement techniques and tools.​ Coordinates with Trinity Health System Office in the execution and implementation of initiatives. Provides input to directional decisions that have system-wide impact.
Strategic Support & Accountability
Responsible for state and regional integration including service line development and performance excellence.​
Operate as a fully integrated system through realized efficiencies and synergies system wide.​
Works effectively with RHM leaders to connect home health into clinically integrated networks. Ensures that services are provided in a manner that supports continuous quality improvement, customer-oriented focus and quality client care outcomes.
Accountable for the selection, evaluation and overall success of the senior leadership teams within the market(s).
Operational Delivery
Drives THAH goals, plans and targets. Oversees and leads the attainment of annual operating goals, plans and financial targets through the THAH leadership team.
Provides direction for THAH Home Based Services' operational performance by monitoring the attainment of annual operating goals, plans and financial targets and works with direct reports to develop corrective strategies as needed.
Provides executive leadership over the development and implementation of standardized policies/procedures and processes, quality measures and overall controls which guide operations of the home care and hospice programs. Ensures policy and procedure support the strategic objectives of the organization.
Approves the allocation of resources (e.g., financial, people, technology) across THAH including the THAH financial plans and forecasts in collaboration with the executive team.
Ensures organizational compliance with applicable laws and regulations and follow up on all legal actions affecting THAH, working with the Trinity Health Legal Department as appropriate.
Maintains a working knowledge of applicable Federal, state & local laws/regulations, Trinity Health Integrity & Compliance Program & Code of conduct, as well as other policies & procedures in order to ensure adherence in a manner that reflects honest, ethical & professional behavior.
Minimum Qualifications
Must possess a Bachelor’s Degree in a related field. Approximately 10 years of progressively more responsible related work experience that includes the development and implementation of programs in home care and hospice services.
Additional Qualifications (nice to have)
A Master’s degree is preferred in Health Care/Business Administration or other closely related field.

Includes the need for continuous physically & mentally capability to perform assigned processes.
Includes work time that involves occasional physical efforts (e.g., transporting, moving, positioning & ambulating patients).
Includes daily activities that require occasional standing / walking with the ability to vary / adjust physical position or activity.
Includes occasional ability to lift a maximum of 30 pounds unassisted, use upper & lower extremities, engage in frequent bending / stooping / reaching & pushing/pulling with infrequent (rarely) climbing, kneeling, crouching, or operating foot controls.
Indirect / Healthcare Support Services:
Occasional clinical / patient facing work environment.
Includes frequent work time indoors (subject to travel requirements) under temperature-controlled & well-lit conditions; may encounter occasional variable external environmental conditions.
Includes occasional lifting, up to 30 pounds unassisted, frequent travel & use of computer; Includes frequent sitting & may require occasional long periods of continued walking, standing, stooping, bending, pulling & pushing.
Direct Healthcare & Indirect / Support Healthcare Services: must be able to:
Adapt to continuous interruptions, shifting priorities & stressful situations; Attend to tasks until complete, understand & relate to complex ideas & concepts & be able to remember multiple tasks & regimens extending over long periods of time & work on concurrent tasks / projects.
Read small print, continuous ability to hear normal sounds & voice patterns, able to give / receive instructions & other verbal communications in-person & over the phone / computer / device / equipment assigned with some background noise.
Perform constant manual dexterity activities & occasional grasping/handling.
Continuous attention to maintain a safe working environment & use of available personal protective equipment (PPE).
