Shift: Monday through Friday – normal business hours. No nights, No weekends, No holidays

Type of Work:

• Follow-up, communication, and development of care plans for patients with chronic illnesses
• Partnering with patients and providers to help achieve the highest level of wellness
• Work in an EMR system daily entering health data

Requirements:

• 2+ years of clinical hospital experience
• Registered Nurse ( RN )
• Recent clinical assessment skills
