Physical Requirements
This position requires occasionally sitting at a computer workstation for extended periods of time; and performing tasks with repetitive motions (such as typing); and standing or walking with objects weighing up to 50 pounds. Also involves some exposure to loud noises, temperature extremes, hazardous chemicals and fumes including waste, blood-borne or airborne pathogens, and combative/violent people.

Shift
Monday through Friday; 8:00 am – 5:00 pm.

Job Summary
This position will manage daily operations, troubleshoot areas of concern, manage program policy and procedures, monitor program goals, work with the Director of Population Health, Nurse Manager and Program Support personnel regarding HR/onboarding/retention and manage the budget as necessary with the current and continued expansion of population health. The practice manager will help with strategic planning and quality improvement work among the population health programs to drive positive patient outcomes. This position will also have direct oversight of programs and employees.
This is a hybrid position; remote work and on-campus attendance expected.
This position has been identified as supporting UK HealthCare, which receives funding from the Centers for Medicare and Medicaid Services (CMS). Therefore, all individuals hired into this position must comply with the federal CMS mandate issued in March of 2022. This mandate requires all new hires to provide proof of COVID-19 vaccination or obtain an approved medical or religious exemption as a condition of employment. Start of employment cannot occur until two weeks after receiving the full COVID-19 vaccination series or upon obtaining an approved exemption. Only vaccines approved by the World Health Organization can be accepted.
