The Oregon State Hospital (Salem, Oregon campus) is searching for a Physical Therapist (PT) to join our Treatment Services Team.
The Oregon State Hospital, a JCAHO accredited and CMS certified public psychiatric hospital, provides psychiatric hospital services and residential treatment services to persons committed by the Oregon courts as part of the state mental health system.
This position is within the Treatment Services Department at Oregon State hospital, which provides evaluation and treatment to persons with mental illness. Treatment Services provides patient evaluation and treatment based on professional standards in occupational therapy, therapeutic recreation, physical therapy, art therapy, and music therapy. Approximately 730 patients are served at two hospital sites.
What will you do?
This position provides services and active treatment for patients admitted to Oregon State Hospital. The primary focus of this position includes providing person-centered recovery-focused physical rehabilitation services, standardized assessments, developing evidenced-based clinical interventions to meet individualized treatment needs.
Assessment:
  • Complete clinical assessment within timelines as established by OSH policies and Treatment Services protocols.
  • Provide consultation to unit team members regarding completed patient referrals.
  • Meets the patient’s goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with ordering practitioners.
  • Complete clinical assessments in accordance with the OSH, Treatment Services, and professional standards.
  • Determine physical therapy treatment goals with client input.
Program planning and implementation:
  • Evaluates effects of physical therapy treatments and fit of prosthetic and orthotic devices by observing, noting, and evaluating patient's progress; recommending adjustments and modifications.
  • Develop and implement therapeutic treatment plans in consultation with clients and ordering practitioners.
  • Helps patient accomplish treatment plan and accept therapeutic devices by administering manual exercises; instructing, encouraging, and assisting patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily-living activities and in using assistive and supportive devices, such as crutches, canes, and prostheses.
  • Administers physical therapy treatments by giving massages; initiating traction; applying physical agents; utilizing hydrotherapy tanks, moist packs, ultraviolet and infrared lamps, and ultrasound machines.
  • Demonstrate creativity and skill in developing a treatment plan that recognizes and builds on the strengths of each patient.
  • Develop and write program plans for each group provided and treatment service, based on identified patient’s strengths and needs according to Treatment Services and OSH guidelines.
  • Directing treatments given by aides, technicians, and assistants.
Documentation and program reporting:
  • Regularly evaluate efficacy of clinical interventions.
  • Track and report clients’ progress, regress or stability toward their individual treatment goals.
  • Document interventions and services according to OSH policies.
  • Reassess client treatment goals by modifying, adapting, and progressively updating the Treatment Care Plan as an active member of the Interdisciplinary Treatment Team.
  • Completes discharge planning by consulting with physicians, nurses, social workers, and other health care workers.
  • Utilize standardize measuring tools when available.
  • Promotes continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs as needed.
  • Involve the client in the planning and goal setting of the individualized treatment plan.
Quality improvement:
  • Utilize Lean Management techniques and systems.
  • Track metrics as assigned.
  • Engage in OSH and Treatment Services Process Improvement activities.
  • Submit written evaluations of program effectiveness to Director of Occupational and Physical Therapy as requested.
  • Participate in regular curriculum development planning by discussion, development of programming, outcome measures, etc.
Infection control and safety:
  • Protects patients and employees by adhering to infection control policies and protocols.
  • Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs.
  • Maintains safe and clean working environment by complying with procedures, rules, and regulations.
Professional Development:
  • Maintain all job-related credentials, licenses, and certifications; reporting change if necessary and renewal documentation when completed.
  • Initiate and attend at least one continuing education opportunity offered outside of OSH annually.
  • Complete annual education e-learnings, Safe Together and CPR updates within due date.
  • Assume personal responsibility for ongoing continuing education and professional development.
  • Develops and demonstrates competencies and practice of the Collaborative Problem Solving® model through formal coaching, training, and development opportunities. Applies the Collaborative Problem Solving® model with clients and staff at all levels of the organization.
  • Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies; and organizing and presenting at least one in-service to the Occupational and Physical Therapy department per year.
  • Attend departmental in-services and meetings, unless excused by Associate Treatment Services Director or designee.
  • Actively participate in individual and group clinical supervisions at minimum once per month with Clinical Supervisor or designee.

MINIMUM QUALIFICATIONS

A license with the Oregon Physical Therapist Licensing Board as a Physical Therapist; OR completed an accredited physical therapy educational program and be eligible to obtain an Oregon Physical Therapy license or temporary permit.
