As a Diabetes Care Coordinator you will be responsible for supporting pharmacists and educators in clinical programs offered to patients for ongoing diabetes self-care management.
Responsibilities:
  • Ability to trouble shoot issues by telephone that may occur with medical equipment
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
  • Abides by all regulations, policies, procedures and standards
  • Courteously assists all customers, internal and external
  • Perform meter training sessions and access additional needs for on-going diabetes training and support by telephone for diabetic patients
  • Contacts doctors’ offices to request new prescriptions for pharmacy patients
  • Performs inbound and outbound patient enrollment calls to diabetes wellness program along with continued support after initial delivery of medical equipment and/or supplies
  • Answers inbound patient-related calls, when needed
  • Maintains a working knowledge of assigned specific pharmacy area/functions as well as cross training in other areas throughout the pharmacy as indicated by management.
  • Maintains a high degree of confidentiality at all times due to access to sensitive information
  • Achieves stated goals and objectives as outlined by managements
  • Follows all Medicare, Medicaid, HIPAA, and Private Insurance regulations and requirements
  • Possesses a strong pharmacy knowledge base including translation of sig codes, product selection, basic knowledge of brand and generic drugs by name
  • Assist pharmacists and certified diabetes educators with patient survey questions regarding diabetes disease management.

  • Ability to access if patient has additional needs for on-going diabetes education and coaching
  • Proficient knowledge regarding diabetes disease state and all associated medical equipment and supplies
  • Proficient in training diabetic patients on using blood glucose monitor
  • Performs administrative functions such as scheduling delivery of medical supplies
  • Ability to guide a patient through how to use their medical equipment and the key-features
  • Maintains a complete understanding of prescription processing including: data entry, creation of shipment schedules, verification of insurance and patient eligibility, prescriber calls for prescription refills, intake of refill Rxs from prescriber, generation and collation of processed queues
  • Achieves stated goals and objectives for patient engagement
  • Responds to patient service-related questions and/or concerns
  • Maintains a working knowledge of assigned specific pharmacy area/functions as well as cross training in other areas throughout the pharmacy as indicated by management

Qualifications:
  • Must be flexible regarding work schedule. All positions require evening and weekend availability.
  • Will be required complete AADE training for a paraprofessional level II and complete ongoing training as directed by supervisor
  • Ability to deal with problems involving several concrete variables in standardized situations
  • Must practice and promote Policies and Procedures, Mission Statement, Values and Goals.
  • Prefer basic understanding of diabetes, diabetic care, diabetic medications, glucometer management, finger stick blood sugar testing, insulin administration and be able to recognize and act upon symptoms of hypoglycemia and hyperglycemia as indicated per State regulations.
  • High School diploma or GED with a minimum 2+ years working as a Pharmacy Technician (inbound/Outbound call center experience preferred) required

  • Certified Pharmacy Technician is preferred
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form.
  • Must have the ability to remain calm under stressful situations.
  • Must maintain in good standing registration as a Pharmacy Technician with the state
  • Maintain all required continuing education for licensure and certification
  • Strong multi-tasking skills and attention to detail
  • Must have compassion for and desire to work patients. Must have basic computer skills or willing to learn computer skills needed for e-MAR system.
