Job Description
The Patient Navigator Coordinator will work under and in coordination with the Oncology Shared Services Nurse Navigator to support and execute Shared Services Telephonic Clinical Network programs. The Patient Navigator will provide telephonic outreach to patients for multiple reasons, depending on program specifics and assist in ensuring support and resources for the patient by coordinating with designated practice contacts. The Patient Navigator will navigate through multiple electronic systems daily populating all data as directed. The Patient Navigator will follow all McKesson ICARE and ILEAD policies .
Key Responsibilities (List the top five to seven essential responsibilities in priority order.)

  • Coordinates patient needs for appointments and schedules appropriately to expedite the plan of care.
  • Works under direct supervision and in tandem with the Oncology Shared Services Nurse Navigator to support and execute Shared Services Telephonic Clinical network programs.
  • Supports patients through coordination of necessary clinical appointments and conveying information about internal and external resource availability.
  • Supports the Oncology Nurse Navigator in reporting patient and family challenges and assists with potential solutions, while considering the patient’s values and cultural, spiritual and social needs.
  • Educates patients, families and caregivers about the role of both the Patient Navigator and the Nurse Navigator.
  • Appropriately documents all patient interactions and services provided within the electronic medical record.
  • Navigates daily through multiple Electronic Medical Record systems and internal systems specific to individual programs.
  • Facilitates communication among health care providers.
  • Follows and abides by remote employee guidelines.
  • Participates in all other job assignments needed to support Oncology Nurse Navigator and other Shared Services supervisors, managers or director.
  • Participates in team and department meetings, educational sessions and events.
  • Follows McKesson ICARE and ILEAD policies
  • Maintains patient privacy according to local and national laws, treating all patient and family information as privileged and protected.

Minimum Requirements:
Education/Training –
  • Minimum of 3 years working as a Certified Medical Assistant
  • Medical Assistant Certification

Additional Skills:
  • Oncology experience preferred
  • Must be able to join all training, meetings and conversations with coworkers and management online with laptop camera and audio on.
  • Must be able to work remote and have dedicated space for workstation and high-speed internet in place.
  • If local and within 1 hour of Woodlands office will be required to come into office setting for monthly or quarterly meetings.
