Job Description
JOB SUMMARY:
Works in conjunction with the Community Health Centers and the Patient Care Team to help improve the health status, health knowledge, attitudes, and practices of patients by promoting, supporting, and assisting with follow-up calls, health assessments, home assessments, vital sign measurements, diabetes blood testing, non-emergency medical transportation, prescription delivery, health education and prevention. Frequently travels to assigned five-county service areas. Works with the elders, children, and others as necessary in the five-county service area. Supports the Project Coordinator for the Special Diabetes Program for Indians (SDPI) grant to assure all deliverables are met in a timely manner and costs are aligned with the scope of the project.
JOB FUNCTIONS:
include, but not limited to, the following:
  • Coordinates with the SDPI Grant Project Coordinator on the Special Diabetes Program for Indians to include program and budget development/monitoring, timely reporting, assuring goals and objectives are met, implementing program activities and compliance with all special grant conditions.
  • Responsible for assuring medications are delivered in a timely manner.
  • Report health concerns back to the Patient Care Coordinator.
  • Other duties as assigned to support the efficient operations of the Health Department.
  • Coordinates home visits with the Patient Care Coordinator and medical provider.
  • Delivers high quality customer service.
  • Makes scheduled home visits to our designated services areas and preforms vital signs, diabetic blood testing, provides general health education, and makes referrals.
  • Transports clients for non-emergency medical services.
  • Participates in community functions and events as part of the Health Team.
  • Assists with community health outreach screenings.
  • Assists with community health education, promotion, special projects, and disease prevention events.
  • Coordinates with the Outreach and Enrollment Specialists to help enroll clients in programs for services.
  • Enters patient contact and patient data into the patient’s chart (AthenaHealth).
  • Attends training and conferences to meet requirements for maintaining certification.
  • Provides education in the use of such things as: walkers, crutches, wheelchairs, canes, and other applicable durable medical equipment (DME).
  • Assists with patient Activities of Daily Living and Instrumental Activities of Daily Living as determined by Patient Care Coordinator and medical provider.
  • Requires frequent travel to fulfill job responsibilities.
Requirements
General Requirements:
  • Must pass a criminal and fingerprint background check according to P.L. 101-630 standards required.
  • Must pass the patient care credentialing process required.
  • Must pass a pre-employment alcohol/drug screening required.
  • Must pass a physical examination prior to or contemporaneous with hiring.
  • Must be insurable through the Tribe’s vehicle insurance required.
Education & Experience:
  • High School Diploma, equivalent GED or higher required.
License & Certification:
  • Must possess, maintain, and provide proof of a valid Driver’s License required.
  • Must obtain, maintain, and provide proof of a valid Community Health Representative (CHR) Basic Training Certificate within three (3) months of hire.
  • Must obtain, maintain, and provide proof of a valid Basic Life Support (BLS) Certificate within three (3) months of hire.
