Aegis Health Care is currently seeking a detail-oriented Patient Care Coordinator to be part of our Atlas Medical team. Atlas Medical specializes in providing primary care for geriatric patients in Senior Living communities. This is a full-time position working Monday-Friday 8am-5pm in our Mesa, AZ office.
The Patient Care Coordinator (PCC) is an important part of the Aegis Healthcare team. Aegis Health Care specializes in providing complete care for geriatric patients. The PCC provides planning, direction, and coordination of overall healthcare of residents in senior living communities. You will collaborate care between the facility, family, and providers that serve the patient.

Job Responsibilities
  • Ensure compliance with federal, state, and local regulations and standards
  • Coordinate schedules of residents and providers to maximize efficiency
  • Ongoing communication with the patient, patient's family, provider, and facility keeping them informed of all medical needs
  • Accept and perform other related duties and responsibilities as required
  • Collect personal health information and input it into the patient medical record
Skills & Qualifications
  • Criminal background check clearance/fingerprint clearance
  • 1-2 years medical experience preferred
  • Compassionate and caring attitude
  • Service oriented mindset
  • Adaptive problem-solving skills
  • CNA or MA license preferred
  • Negative TB test
  • Valid driver's license, clean driving record, and reliable automobile
  • Ability to relate well to staff and other professionals
  • Proof of current automobile insurance
