5. Dash cam (front view non audible) $70.00/mo. / $150.00 deposit.

7. Plates and registration ( prorated) $150.00 filing fee / 2 installments.

8. Insurance+Cargo (1 million liability) $1100.00/ monthly

Please be advised that some of the above services are billed weekly and others monthly.

_ OWNER OPERATOR REQUIREMENTS_
Owner operator with truck and trailer(flatbed-step deck) under CBFI AUTHORITY.

1. Valid CDL A and Medical Card.

2. Good MVR and PSP

3. Registered in a clearinghouse.{CLEAR)

4. No more than 3 TICKETS and 1 NOT AT FAULT accident.

5. EIN and Corporation articles 5.1 or IRS letter.

6. Verifiable References and loads pictures.

5. Must have a minimum of one year experience on a flatbed.

6. Must have knowledge of freight securement.

7. Must have at least 2 years of recent CDL A OTR driving.

8. Able to pass a pre employment drug test.

9. Able to drive to our terminal in Illinois.

10. Attend an orientation in our office.

11. NO DUI -DWI-SAP-SUSPENSIONS-ACCIDENTS WITH INJURIES-ROLL OVER-RECKLESS

DRIVING- FELONIES OR MISDEMEANORS.
