Aegis Home Health and Hospice is currently seeking a compassionate Registered Nurse (RN) to be an on-site coordinator. This is a full-time position working Monday-Friday 9am-5pm in Brigham City.
Each Registered Nurse is an important part of the Aegis Health Care team. Aegis specializes in providing care for hospice and home health patients in senior living communities or in the patient's home. The Clinical Care Coordinator collaborates with the Director by providing management, supervision, planning, direction, and coordination of overall healthcare of residents in our partner communities that elect our wellness program. Job Responsibilities
  • Set priorities, coordinate, and organize patient care delivery through effectively managing time, supplies, and resources
  • Actively seek patient, family, team, and physician involvement to develop plan of care including needed education
  • Assist the partner community in any coordination of care that the patients may need
  • Initiate referrals as needed to other health team members
  • Participate in team meetings and coordinate patient care with other members of the team
  • Use assessment information and critical thinking skills to develop an individualized plan of care; revise and reassess changes as appropriate
  • Perform appropriate assessment of physical, social, and psychological status
  • Maintain patient records showing systematic assessment, planning intervention, and evaluation
Skills And Qualifications
  • Current CPR certification
  • Negative TB test
  • Current Registered Nurse (RN) license in state of practice
  • Ability to communicate effectively both verbally and in writing
  • Proof of current automobile insurance
  • Criminal background check clearance/fingerprint clearance
  • Valid Driver's license, clean driving record, and reliable automobile
  • Home Health / Hospice experience as a Registered Nurse (RN) preferred
  • Ability to utilize full range of movement to transfer patients
