Overview
The Oncology Stewardship Pharmacist is responsible for oncology formulary management including onboarding new agents including determination of site of care (inpatient, outpatient, regional campuses); oversight of the review process for off-label, non-formulary or other non-standard use of antineoplastic agents and collaboration with pharmacy IS teams on development and revision EHR resources. The oncology stewardship pharmacist will also participate in the promotion of appropriate antineoplastic utilization through staff education and monitoring of antineoplastic use practices.
This position’s work location is partial remote with two or three days per week remote. The selected candidate may only work remotely from a New England state (ME, VT, NH, MA, CT, RI).

Responsibilities
Coordinate the onboarding of new antineoplastic medications including formulary process, IS process and operations by ensuring a medication follows all the necessary steps from formulary request by a DFCI provider through pharmacy IS and COE builds and staff education and implementation with nursing and pharmacy teams.
Ensure standardization of medication onboarding new drugs and implementation of antineoplastic related guidelines across all DFCI sites where appropriate.
Participate in OSL medication standardization work group to ensure DFCI representation in clinical, IS and operational decision making and participate in implementation of build changes through the active patient management process and promotion of clinical support tools where appropriate.
Review provider requests for exception orders and evaluate for compendia or literature support to guide the prior authorization process.
Contribute to the development and maintenance of policies and procedures to support the safe use of antineoplastic medications across all DFCI sites and promote and enhance patient outcomes through safe, effective, cost-efficient, and appropriate medication therapy use for patients receiving antineoplastic therapy.
Contribute to the institution’s antineoplastic stewardship program through compiling data and metrics on antineoplastic use in the ambulatory setting and reporting data and finding to relevant stakeholders.
Build and maintain databases and dashboards that support the work of the Antineoplastic Stewardship Program as required.
Collaborate with the Department of Quality and Patient Safety in the assessment, mitigation and dissemination of antineoplastic-related safety initiatives.
Coordinate, facilitate, and promote education, training, competency assessment and performance improvement of pharmacists, pharmacy residents and other healthcare professionals in the appropriate use of antineoplastics.
Collaborate with other disciplines to develop and implement best practices across the institute related to antineoplastic utilization.
Qualifications
The Oncology Stewardship Pharmacist must meet the following requirements to be hired and maintain their position in the Pharmacy Department at DFCI.
Minimum Education:
All Pharmacists require a Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of Pharmacy.
Minimum Experience:
5 years of clinical pharmacy experience with 4 years in oncology required. Oncology Residency preferred.
License/Certification/Registration:
Must be licensed to practice pharmacy in the Commonwealth of Massachusetts. An advanced degree related o specialty practice or Board of Pharmaceutical Specialties or other relevant certification. Must complete 20 continuing education credits per year to maintain licensure.

Demonstrated ability to establish and implement a comprehensive regulatory oversight and outreach program in a complex organizational structure.
Proficient in Microsoft Office Suite (Word, Excel, Outlook and PowerPoint).
