BMT ONCOLOGY CLINICAL PHARMACIST
- (3242690)GENERAL SUMMARY/ OVERVIEW STATEMENT:
Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services in a practice setting with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management within specialty patient care services and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events.
Clinical Pharmacists competently manage and facilitate the appropriate use of medications according to professional and regulatory standards. Clinical Pharmacists provide medication and practice-related education/training and actively serve as preceptors for pharmacy residents and Doctor of Pharmacy students.
The BMT Oncology Clinical Pharmacist is an integral member of the Department of Pharmacy and Cancer Center, serving as a resource and liaison for the ambulatory and inpatient oncology practices. The BMT Oncology Clinical Pharmacist works in close collaboration with multiple role groups within the MGH Cancer Center. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Maintains compliance with all BMT program accreditation pharmacy standards
- Promotes a culture of safety; adheres to established practices and policies of the department
- Direct patient and medication management
- Precepts learners (students, PGY1 Residents, PGY2 Residents). Meet Preceptor requirements as outlined in the MGH Residency Preceptor Responsibilities, Credentials, and Development Policy
- Prepares for BMT program accreditation visits
- Direct support of Pharmacy operations including order verification, processing, checking, and dispensing for patients
- Provides drug information to other health care professionals
- Participates in and identify quality improvement projects
- Plans patient treatment in collaboration with multidisciplinary care team
- Serves as drug information resource and educate staff including pharmacy, nursing, and providers
- Completes and submits reports including Safety Reports; participates in the Medication Safety Process
- Maintains key compliance (e.g. CAR-T REMS compliance, TECVAYLI REMS compliance, FACT compliance with pharmacy standards, TJC accreditation readiness)
- Serves as pharmacy champion and liaison for designated disease group
- Actively participates in and contributes to the MGH BMT Quality Committee and patient planning meeting
- Design, management, and optimization of Beacon Treatment Plan Templates available to designated disease group
- Patient encounters either at the bedside, in the clinic, and/or virtually
- Participates in the training and mentorship of new employees, students, and residents
- Manages off-label chemotherapy requests and nonformulary requests
- Supports Pharmacy Department through active participation in Pharmacy Collaborative Governance Committee
- Supports MGH Cancer Center Network through participation in Key Cancer Center Committees
QUALIFICATIONS:
REQUIRED:
- Licensed to practice pharmacy in Massachusetts
- A PGY1 Pharmacy Residency or equivalent experience
- Board Certification in Oncology required within 18 months of hire
PREFERRED:
- Work experience may be considered in place of PGY2 Pharmacy Residency
- A PGY2 Oncology Pharmacy Residency Graduate
- Work experience may be considered in place of PGY2 Pharmacy Residency
