Job Description
Job Summary: The primary purpose of this position is to provide Occupational Therapy services to include: the developing, improving and/or restoring the performance of Activities of Daily Living (ADL) functions, compensate for dysfunction, and/or minimize debilitation, and the planning for and documentation of treatment performance for all resident requiring these services.DUTIES AND RESPONSIBILITIES:Demonstrates Competency in the Following Areas:Demonstrates working knowledge of skilled restorative therapy and maintenance programsProvides Occupational Therapy plan of treatments and modalities per physician ordersMust possess the ability to deal tactfully and effectively communicate with personnel, residents, family members, visitors, and the general publicMust effectively communicate with residents with visual, speech, auditory, cognitive, physical and emotional impairmentsMust possess the ability to train/supervise and discipline subordinate staffMust be able to relate information regarding a resident’s conditionMust possess the ability to assist in planning, organizing and implementing the goals, objectives and policies and procedures of the departmentDocuments resident progress toward occupational therapy goalsCompletes Daily Activity Schedule and progress notes per facility policy.Provides resident, staff and family education as directedComplies with Federal and State regulations related to rehabilitation therapy Completes all documentation to comply with professional standard and regulatory requirements Strives to meet productivity / efficiency goals and self-initiates corrective action plan for non-attainmentUtilizes the clinical software applications per policies and proceduresAssists in ordering necessary equipment for the facility and residentsUtilizes proper body mechanics, techniques and safety while completing job duties and responsibilities.Maintains current professional knowledge of practice of Occupational Therapy and practices within acceptable professional standardsComplies with all facility policies and proceduresProfessional Requirements:Adheres to dress code, appearance is professional, neat and clean. No long artificial nails, no large or long dangling jewelry, no large dangling earrings.Completes annual education requirements.Maintains resident confidentiality at all times.Reports to work on time and as scheduled, completes work within designated time.Wears identification while on duty, uses computerized punch time system correctly.Completes education/in-services in a timely fashion. Monthly and annually and as requested.Attends annual review and department inservices, as scheduled.Attends staff meetings such as shift huddles/reports.Actively participates in performance improvement and continuous quality improvement (CQI) and QAPI activities.Complies with all organizational policies regarding ethical business practices.Communicates the mission, ethics and goals of the facility.Makes eye contact, smiles and greets others by nameMake an effort to get to know team members, residents and their families.Learn about and deliver on the needs of the residentsDo what you say you will doFollow up in a timely mannerTake time to listen to and understand othersRecognize and share successes of our Life in Balance culture in actionBe willing to help othersReach out and ask for help when neededSpeak positively about our care center and the people who live & work hereBring positivity and fun to the work environmentAccept and learn from mistakesBe flexible and adaptive to changes in the work environmentSeek to find solutions to problemsTake responsibility for the quality of your workSeek out information you need to be your bestShare your knowledge and expertise to help othersBe knowledgeable and enthusiastic about our care center and what we doCommunicate and work together across departmentsRegulatory Requirements:Must possess a current unencumbered NYS license/Certification as an Occupational Therapy AssistantGraduate of an accredited school with an Associate’s degree in Occupational Therapy.Language Skills:Able to communicate effectively in English, both verbally and in writing.Additional languages preferred.Skills:Geriatric experience preferredA current unrestricted driver’s license and dependable transportationAble to use necessary computer applicationsAbility to professionally communicate with internal and external resourcesHigh level of motivation and ability to take initiative appropriatelyAbility to function independently as well as part of a teamAbility to function effectively in stressful and changing situations Ability to positively give and receive oral and written direction Physical Demands:In good physical healthAble to push wheelchairs/Geri-chairs with residents Able to push supplies on carts weighing up to 50 poundsAble to walk, bend, stoop, kneel, and climb/descend stairs throughout shiftAble to reach above and below shoulder heightAble to lift and carry supplies and equipment weighing approximately 25 pounds
