Job Description
Job Summary: Work as a member of the Nutrition Health Education (NHE) team to engage HIV+ individuals who have unmet nutritional needs and/or who are at risk for nutrition deficits to improve medical outcomes through nutritional assistance, coordinate with the Program Manager to appropriately administer the NHE grant funding and ensure operational integrity of the program. Activities include; connecting PLWHA to other supportive services, conducting outreach activities to facilitate enrollments, tracking and documentation of Nutrition Health Education participation and services, as well as coordinating with the Program Manager and other agency staff to fulfill purchase order, track the NHE budget, and program spending.
Major Activities
  • Attend agency Leadership Meetings.
  • Complete required documentation in client charts, program logs, and AIRS in accordance with policies and procedures and funder requirements.
  • Conduct recruitment/program promotion outreach with the target population and community service providers to engage clients in services.
  • Perform other tasks as assigned.
  • Attend all group nutrition education sessions to confer with clients to identify there sources necessary to meet their needs. Confer with each client at least monthly to ensure that client needs are identified and the appropriate referrals are made. Make referrals, verify access to services, and address barriers as needed.
  • Provide support to food pantry/grocery bag and food gift card/voucher distribution services. Monitor and log food voucher usage and client grocery receipts to ensure that food purchased meets RW standards. Provide feedback to the Program Manager and NHE as needed.
  • Drive program van.
  • Incorporate the Transtheoretical Model of Behavior Change, as applicable, into all aspects of programming.
  • Complete purchase orders for NHE program needs, monitor/track the NHE budget and program spending.
  • Incorporate best practices regarding confidentiality into all job duties and communications in accordance with HIPAA, Article 27-F, Alliance for Positive Health policies and procedures and other applicable regulations. Observe and abide by the HIV Confidentiality Law.
Qualifications
  • Associates degree in health or human services; Bachelor’s preferred
  • 1-2 years’ experience working in HIV/AIDS or within chronic illness setting
  • Must have valid, unrestricted driver’s license and have a vehicle to travel to designated appointment sites within a multi-county region.
Employment Classification: Part-time, Non-exempt. 22.5 hours per week; 7.5 hour work days M, Tu, W; flexibility required; $18.50/hour starting pay with minimum qualifications.
