OrderNotice: Hartford County positivity rate over 5%, community will require spit test upon arrival regardless of vaccination status. Please have the employee bring a copy of insurance card. They will have to sign a consent form and see the LPN on shift to guide them through the test process.

COVID testing is not required for Vaccinated associates unless symptomatic or if there is an exposure/outbreak

Unvaccinated associates require weekly PCR or Saliva test. Results must be in hand and brought to the community. Agency employees may not work in the community until the negative result is received. An attestation confirming this initial negative result is not acceptable to begin.
