Position Details: Part-Time Day Position Heart Care Specialists of St. Luke's Medical Group Description:
To work collaboratively with a multidisciplinary health team, under the medical supervision of an HEART FAILURE SPECIALIST, to provide comprehensive care for both well and ill patients. Major duties and responsibilities are established by the Nurse Practice Act of Missouri. Performs duties and responsibilities in a manner consistent St. Luke’s Medical Hospital.

Collaborate with physicians in the complex and labor-intensive management of heart failure
Maximize efficiency and cost-effectiveness
Decrease hospital readmissions
Maximize quality of life for patients with heart failure
Educate patients

While oversight is provided by Dr. LaRue, the NP will be the healthcare provider at the clinic and their duties will vary (serving as clinician, care coordinator (schedules appointments with other providers (nutrition, dietary), scheduler, etc.
NP will mine data along with Case Managers in Pop Health via Cardio Care and other Health E Registries to capture any patients that are not being followed per heart failure guidelines and to coordinate with the patient’s PCP, Cardiologist, etc.
Clinic Time
Assesses the physical and mental condition of patients by performing and obtaining comprehensive or problem focus physical exam and medical history.
Orders, collects, interprets and evaluates lab and other diagnostic test to assess patient problems and health care needs.
Formulates and implements comprehensive outcomes-based patient treatment plans based on patient assessments, evidence-based medicine, standards of care, professional practice guidelines and in collaboration with physicians and other interdisciplinary team members when appropriate.
Counsels’ patients and families regarding medical processes, management of the health illness, medical condition, medication, nutrition. Counsels’ patients about drug regimens.
Maintain appropriate records detailing the patient’s treatment plans and outcomes. Initiates timely consultation and referral when the problem exceeds scope of practice and/or expertise.

Facilitates patient movement through the health-care system and efficient use of resources through collaboration, as well as interaction with inpatient and outpatient care providers within and outside our healthcare system.

Supports research efforts to advance knowledge and to promote research-

Specific tasks related to Outpatient office clinic:
1. Responding to phone calls from patients and referring MD's.
2. Assisting with direct admits from office as requested,
3. Perform comprehensive patient assessment including physical examination for follow-up visits and same day urgent follow-up. Collect and evaluate diagnostic data. Assure necessary medications and other therapies are prescribed in collaboration with a physician
4. Formulate plan of care in collaboration with patient and Heart Failure MD. Educate patient and family in order to promote wellness, prevent health problems, maintain current health status and intervene in acute and chronic illness. Evaluate patient and family outcomes and modify plan of care in consultation with physicians and other health team members
5. Work with any clinical programs to identify patients that are not meeting the standard of care for Heart Failure, discuss with MD and determine course of action.
5. Work with office staff to place orders for hospital procedures, labs and medications.

Total 100%
Education:
Current licensure as a Registered Nurse in the state of Missouri Required.
Nursing Diploma/Associates/Bachelors in Nursing Required.
Licensed Adult Nurse Practitioner Required.
Experience:
Cardiac background required
Heart Failure (HF) background preferred.
