Job Description

Duties and Responsibilities

Adult Day Care Services:

  • Creates and maintains a caring, safe and clean service environment for the Adult Day Center.
  • Assist in daily recreational activities.
  • Conducts monthly health education and nutrition workshops for consumers.
  • Monitor and document participants’ progress, changes and matters of concern in the consumer files via case notes and communicate information to key parties (Supervisor, Case Manager and/or caregivers).
  • Assist participants with ADL’s and/or IADL’s as needed.
  • Responsible for the handling, dispensing, storing, and disposal of consumer medication.
  • Evaluates consumer daily medical needs, identifies health concerns, and documents findings and interventions.
  • Complete assessments, service plans and all other documentation as needed as per agency and other regulatory body requirements.

Human Resource Management:

  • Provides on-going supervision to each staff under the incumbent’s supervision.
  • Ensures orientation and training is provided to staff according to regulatory body requirements.
  • Conducts performance evaluations and/or improvement plans for all staff directly under his/her direct supervision.
  • Responsible for assisting in ensuring mandated client to staff ratios.

Risk Management and Quality Assurance:

  • Responsible for following up on all incidents reported timely and accurately.
  • Monitors the compliance of safety and health precautions and monitors staff compliance according to all governing bodies’ policies and procedures.
  • Completes and submits timely and accurate Significant Incident Reports, and any other required documentation as per Catholic Charities guidelines, state licensing requirements, Areawide Council on Aging of Broward County and COA Standards.
  • Must be available to work beyond your regular schedule in crisis situations or if there is a declared agency emergency.
  • Participates in the Performance Quality Improvement process.
  • Participates monthly in the fire drill

Additional Duties:

  • Participates in all related trainings, in-services, and staff meetings as required by COA and funding /licensing sources.
  • Attends mandatory conferences and trainings as scheduled for professional development.
  • Incumbent will comply with all policies, procedures and requirements necessary to perform the functions of this position.
  • Performs other duties as assigned by supervisor.

Physical Demands:

  • The work is usually sedentary, but may require long periods of standing, walking and /or bending and some lifting. The work is mostly performed in a secure office setting.
  • Must be able to lift up to 40lbs in order to assist other staff in lifting or transferring seniors if necessary.

Educational & Experiential Requirements:

  • 1 year of experience working with frail elderly preferred
  • Licensed Practical Nurse in the State of Florida
  • High School diploma or GED
  • Bi-lingual English/Spanish preferred

Reporting to this position:

  • Certified Nursing Assistants
