The Opportunity: Nurse Case Manager - Long Term Care
We are seeking a dedicated Nurse Case Manager with a specialization in Long Term Care to join our dynamic healthcare team. In this role, you will play a vital part in the lives of our patients, providing them with the support they need for their long-term care journey.

What We're Looking For
A nurturing heart and a Bachelor's degree in nursing to show for it.
An unrestricted California Registered Nurse license that lets you practice your passion freely.
Your dependable nature, backed by a valid California driver's license and reliable transportation.
While not mandatory, having a Certified Case Manager Certification would be a plus.
Your hands-on experience as a nurse in an acute care setting, spanning 3-5+ years.
A minimum of 3 years of recent experience in case management, utilization management, or discharge planning.
If you've worked in a managed care setting, that's even better!
Familiarity with Medi-Cal and/or Medicare populations would be a great asset.
A general knowledge of NCQA Standards, while not required, would be appreciated.
