Job Description

Summary:
Review reports and patient charts in OncoEMR to identify all new patients at various office locations. Follow specific guidelines laid out by individual offices/SOP’s. Contact patients by phone to obtain basic demographic information prior to their first visit. Communicate with other departments as necessary to obtain patient information. Send correspondence to the OM of any patients not contacted. Advise the patient of their upcoming appointment with date and time. Maintain accuracy at all times. Follow HIPPA guidelines. All Care Management Specialists are expected to assist with daily departmental and operational tasks including but not limited to handling mail, receiving phone calls and transferring phone calls.
Primary Key Performance Areas
KPA 1 – Product knowledge
  • Utilize resources to answer patient questions.
  • Educate patient about first visit expectations.
  • Provide basic education to all new patients.
KPA 2 – Navigation and Documentation
  • Appropriately document patient communications in OncoEMR.
  • Go through patient EMR chart to obtain demographic information needed prior to first patient visit while maintaining HIPAA guidelines
  • Be able to identify the appropriate actions for a patient based on EMR documentation
KPA 3 – Communication and Customer Service
Ensure effective communication and premier customer service with internal and external customers.
  • Professionally answers all incoming phone lines, accurately navigates the phone system and transfer calls appropriately.
  • Identify when additional information is needed and reach out to the appropriate department to obtain that information in a professional manner as needed
  • Professionally communicates to staff and other departments using Skype messaging, email or via phone.
KPA 4 – Compliance and SOPS
Maintains Standard Operating Procedures of Compliance ensuring requirements are communicated and managed according to accreditation standards, HIPAA, local, state and federal pharmacy regulations.
  • Keeps immediate Supervisor and/or Operations Manager promptly and fully informed of any problems or unusual matters of significance.
  • Complies with CMS regulations and follows Care Management Standard Operating Procedures.
  • Maintain and ensure the confidentiality of all patient and employee information at all times.

KPA 5 – Data Entry
To ensure the accurate and timely documentation of patient information into multiple software systems
  • Responsible for obtaining and maintaining patients up to date information in OncoEMR, Outlook, Excel and Sharepoint.
  • Navigation and documentation in OncoEMR, Outlook, Excel and Sharepoint.
  • Accurately document patient information in OncoEMR in a timely manner as stated by the SOP.
Education: High School Diploma
Previous Experience: One (1) year of experience in the healthcare field or clinic setting with knowledge of oncology preferred.
Office experience to include: procedure entry with experience in insurance and medical terminology preferred.
