Job Summary


  • Provides individuals, families, and groups with the support needed to cope with chronic, acute, or terminal


General Accountabilities


  • Provides client with mentoring, skills building, and individual supportive counseling services in coordination with their family, their community, and other community providers with the goal of assisting the individual with developing and implementing social, interpersonal, self-care, daily living and independent living skills to restore stability, to support functional gains and to adapt to community living.
  • Attends and participates in staff development programs, department related conferences and administrative meetings.
  • Delivers evidence-based services to client and their family in a manner consistent with the program design to assure maximum effectiveness.
  • Reviews and reinforces the individualized treatment plan to reduce risk factors and increase protective factors.
  • Assesses client and family risk and protective
  • Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plan.
  • Counsel's patients to help them overcome dependencies, recover from illness, and adjust to life.

  • Collaborates with medical professionals to evaluate client's medical or physical conditions and to assess client Provides support to client based on these conditions, including transportation to appointments, appointment scheduling, and appointment reminders.
  • Modifies client treatment plans to comply with any changes in status.
  • Refers patients to community resources to assist in recovery.
  • Organizes support groups or counsels family members to help them understand, deal with, and support the patient.
