The Memory Care Program Coordinator will be one of the primary sources of supportive services to residents of the Dogwood community and to residents living in Caleb. This person will be responsible for planning, developing and integrating all aspects of dementia specific care. The Memory Care Program Coordinator will strive to ensure the well-being of residents affected by dementia by assisting in the coordination of services and providing residential support consistent with the mission of Duncaster. Under the supervision of the Director of Life Enrichment the incumbent is responsible for coordinating the development, implementation, and ongoing evaluation of life enrichment and therapeutic recreation programs for residents in Caleb and on the Dogwood neighborhood, Duncaster’s Memory Care Assisted Living community. Incumbent will oversee the development of therapeutic recreational activities designed for the individual and group which stimulate, motivate, and activate leisure interests, wellness and psycho-social well-being of the residents. The incumbent should also maintain a constant relationship with the Life Enrichment team in the sharing of leisure programs and ideas and the delivery of leisure services when appropriate.
The primary purpose of the job position is to plan, organize, develop, and direct the overall operation of programs for residents in Caleb and on the Memory Care Assisted Living community in accordance with current federal, state and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Director of Life Enrichment, to assure that an on-going program of activities designed to meet, in accordance with the comprehensive assessment, the interests and the physical, mental, and psychosocial well-being of each resident is maintained. The secondary focus of the incumbent is to work in relationship with the residents on our Skilled Neighborhoods, Short Term Rehab communities and Memory Care Cedar neighborhood for life enrichment, education, coordination of care, transitions and services.
Duties and Responsibilities
- Responsible for establishing a personalized plan to meet the physical, emotional, social, spiritual and recreational needs of dementia residents. With the team, integrates the routine ADL’s with recreational activities and special events to create a unique atmosphere of caring appropriate to each individual resident. Establishes program goals as directed and communicates them to team members, families and staff.
- Able to work efficiently with other members of the Life Enrichment team.
- Responsible for documentation and evaluation of services received by residents and assists in making adjustments as needed. Demonstrates ability to navigate and understand documentation using the Matrix Server. Communicates clearly with residents, family members and all team members to ensure the highest quality of care.
- Able to manage the community life enrichment budget.
- Able to communicate effectively with staff and family members.
- Understanding of programming of therapeutic recreation activities for the geriatric residents of Caleb and our Memory Care Assisted Living Community. Understanding how to develop craft, art, music, spiritual, exercise and horticultural activities specific to the residents. Able to formulate individualized activity schedule for all residents on a daily basis. Able to evaluate and assess resident behavioral responses in activities and make adjustments as needed.
- Establish and facilitate education and support groups for all residents, staff and families.
- Participate in continuing education to increase knowledge and maintain professionalism.
- Coordinate all move-ins with various departments to ensure a smooth transition. Act as one of the primary sources of supportive services to new residents and families upon move-in and provide an avenue for new residents to feel comfortable.
- Work closely with other departments throughout the continuum, outside service providers, family members and others to ensure residents receive needed services.
- Strives to assure the highest quality of life and services for residents by striving to enrich the lives of the residents every day by providing exceptional living experiences for them through a variety of programs and services.
- Understanding of group dynamics in Caleb and on our Memory Care Assisted Living Community. Able to work within the interdisciplinary team. Able to supervise all residents.
- Attend meetings, serve on committees, and perform other special projects or tasks as assigned.
- Assist in maintaining standards as expected to meet regulatory and accreditation requirements. Maintain confidentiality.
- Lead resource person of dementia care for residents, staff and families.
Job Requirements:
Education and Experience
- Must have ability to read, write and speak English.
- Excellent oral, written, and interpersonal skills coupled with excellent judgment and strong initiative.
- Must possess creativity, strong organizational skills and documentation skills.
- Bachelor’s degree in Therapeutic Recreation and/or Social Work is required.
- Experience with working with individuals with Dementia required.
- Must have strong facilitation and teamwork skills.
- Must be able to respond calmly in crisis or emergency situations.
- 3-5 years of experience with older adults in a professional capacity.
- Must be able to perform essential job functions with or without reasonable accommodations.
- Must show patience, is courteous and caring whenever dealing with residents, family members, staff and prospective residents.
Environment and Working Conditions
- Must be able to cope with the mental and emotional stress of the position.
- Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
- Must be able to push/pull residents weighing 78-250 pounds in wheelchairs or geri-chairs.
- Must be able to speak and write the English language in an understandable manner.
- Must be able to lift/carry/move objects ranging from 10-50 pounds in weight.
- Must be in good general health and demonstrate emotional stability.
- Must function independently, have personal integrity, have flexibility, and the ability to work effectively with other personnel, and agencies.
- Must be able to move intermittently throughout the workday.
