Job Description
Pay: 80,000 to 87,000 annually These are 12-hour shifts. Hospice Nurse (LPN’s & RN’s qualify) We are seeking compassionate and professional Hospice Nurses to join our team. Our Hospice company provides patient-centered care and support to those who are in need of it. We are currently looking for a Hospice Nurses to join our team in the following areas: Naperville, Lake County, Will County, Northbrook, South Chicago, Kane County and DuPage County. : Provide quality of life services to patients in hospice and end of life careReview patient medical records and determine the need for end of life careReview patient preferences and family member preferences to determine the need for end of life carePrepare and provide care to patients in hospice and end of life careMonitor patient’s condition, including weight, temperature, pulse rate, vital signs, etc.Maintain accurate records by recording all patient notes and vital signs in the patient’s record.Conduct periodic visits with patients to review their condition and assure that they are receiving the best possible careCollect all necessary documents from patients for completing reports on visits.Participate in discussions with physicians regarding patient’s condition.Provide emotional support to patients as needed during difficult times such as death of a loved one or diagnosis of a disease.Maintain professional behavior at all times toward other employees, patients, physicians, etc.Perform other duties as assigned by the Director of Nursing Services or Manager of Operations Qualifications:
  • Must be licensed in the state where employed as a Licensed Practical Nurse (LPN) or a Registered Nurse (RN)
  • Must possess current CPR certification (A B) required; possess or obtain current First Aid certification required; possess or obtain current CPR certification (A B) required; other certifications as deemed appropriate by management #plbecky PandoLogic.
