Job Description
Join our team at Castle Peak!We are looking for an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as aPart-Time MDS Coordinator.


In this role, you will manage the clinical care of residents by assigning different tasks to nurses within the department at one of our locations. Our ideal candidate haspriorLTC experience, thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills.


This is aPart-Time,benefit eligible position working 20 to 30 hours per week. It is mostly remote work with 3 to 4 site visits a month.


Pay Range:$30 to $50 / hour depending on experience.


MDS Coordinator responsibilities:


  • Develop initial and temporary care plans.
  • Document and maintain paperwork, referral records, and reports.
  • Work in collaboration with Admissions staff, Case Manager, and Supply Coordinator.
  • Display a courteous attitude and respect for all residents, families, and staff.
  • Assist with floor coverage as needed.
  • Perform additional tasks as needed.
  • Oversee and complete MDS and care planning.
  • Will be part of the management team and do MOD rotation.
  • Assist Case Manager with Medicare scheduling, monthly billing, charting, and issuing notices of non-coverage.
  • Complete all documentation for RAI and MDS assessments and observations.
  • Manage and assign tasks to nursing staff on the assigned unit.


MDS Coordinatorskills and requirements:


  • Minimum of 1+ years’ nursing experience required.
  • Proficient in computer software.
  • Experience with MDS preferred.
  • RN or LPN license in the state ofColorado required.
