We're seeking a talented and friendly MDS Coordinator to provide service excellence to our residents. Our unique 5 STARS service:
- Analyzes and evaluates nursing and related services to improve the quality of elder care and maximize nursing time and abilities.
- Making a difference by providing 5 STARS service: Smile and Greet; Take Initiative, Accept Responsibility; Respect the Whole Person; Seek to Understand.
- Provide the A.D.O.N.s a weekly charting "window" for their forthcoming MDSs as noted by the weekly MDS schedule generated and distributed by HIM.
- Review unit charts daily to ensure the MDS documentation is being entered accurately by the Charge Nurse as required.
- Source, hire, supervise, monitor, evaluate and provide direction to direct reports, as needed.
- Coordinate with the Administrator, Director of Nursing, and QAAC chair-person regarding issues noted on the Facility Quality Indicator Profile.
- Input data onto the RAI instrument either via computer or long-hand in a manner that meets federal and state requirements.
- Ensures compliance with state and federal regulations in all areas of nursing care.
- Maintains awareness of internal operating management policies.
- Getting to know our residents by sharing stories with them while providing comfort, transportation, monitoring vital signs, and assisting with daily living needs.
- Ensuring resident care is the best it can be by following the care plan.
- Conduct in-service training for licensed staff as needed regarding the MDS procedure.
- Administers all intramuscular, sub-cutaneous injections; performs all in-house testing procedures; takes and records vital signs.
- Plans, organizes, and directs nursing personnel in fulfilling their responsibilities for nursing service.
- Check with Health Information Management daily for critical updates.
- Makes daily rounds in the facility for the purposes of visibility, assessment, completion, and evaluation of care.
- Ensures that the individual service plan is followed; administers or supervises treatments prescribed by physician.
As a 5 STARS associate, you'll bring to the team:
- Depth perception and field of vision (peripheral) are not as critical as the ability to distinguish small details and markings very near to the observer. Color perception for some tasks (e.g., dispensing meds, assessment of elder condition) is important.
- Use up to 100 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people.
- Current status as LVN/RN and have an active license
- Ability to communicate verbally with elders, families, visitors and co-workers.
- Must be a graduate of an accredited school of nursing.
- Maintain confidentiality.
- Having a friendly attitude and a servant's heart. (Your friendly attitude and willingness to learn is more important than a year's experience!)
- Work deals mostly with areas such as preparing and reading data and figures, client records, medical orders, visual inspection involving small details, using measurements devices, dispensing medicines. Visual work is primarily close to observer.
- Proficient working knowledge and ability to accurately and timely operate and perform computer related tasks with specific equipment and software applications required.
- Have a current licensure by the Texas State Board of Nursing.
- Effectively manage personnel; requires administrative skills to include but not limited to staff selection, development, motivation, scheduling, and evaluation.
- Ability to read, write and understand business documents which are written in English. Requires ability to speak English proficiently.
