Job Description

Earn $55,000 annually plus 160 hours paid vacation, 12 paid sick days and 10 paid holidays. We offer medical packages and a 401K.
To coordinate intake/admissions process for each minor that enters our program. The Intake Worker will coordinate the initial program orientation, preparation of materials & supervision of minors and documentation needed during the Intake Admissions Process.
HOURS PER WEEK: Non-Exempt
  • Must be flexible to meet program needs; wekeends and holidays required
  • Full time, Overnight, hours may vary
Functions
  • Assemble Placement Authorization and Medical Authorization Form information and brochure are complete with minor demographics and provided to Foster parents
  • Perform other duties as requested by your immediate supervisor or the Chief Executive Officer
  • Responsible for assisting and completing intake tasks, as assigned by management
  • Assure all Property Inventory upon admission arrival is collected
  • Confirm food is available for each intake
  • Create minors admissions STRIDE folder
  • Review and confirm names of minors entering our program and admit them in the UC portal as they arrive at our facility
  • Assess if a minor needs emergency care upon assessment
  • Responsible for maintaining the privacy and confidentiality of Protected Health Information of service recipients in any form (e.g., computer access, conversation, document)
  • Assure completion of minors Admissions Stride Folder
  • Provide, prepare and document Clothing Given upon admissions
  • Facilitate Welcome & Intake Orientation
  • Confirm the number and names of minors expected in admissions daily
  • Provide daily inventory count of clothing and supply usage during intake
  • Perform initial intake assessment and interpret results providing recommendations and information to all departments regarding any important information provided by each minor
  • Assure Property Inventory upon admissions is documented and placed in safe
  • Evaluate any immediate safety concerns for minor and/or agency
  • Respond in a positive and respectful manner to diversity, among the agencys service population and staff
  • Attend/Complete all required trainings
  • Serve as liaison for Foster Home Recruiters and Licensors department and Foster Parents during Intake Admissions. Providing them with documents and orientation of their responsibilities
  • Report intake completion daily
  • Collect and confirm all Foster Parents Placements upon minor admission with Foster Home Recruiters and Licensors Department
  • Assist with rapid COVID-19 testing during Intakes
  • Responsible for complying with rules and regulations governing access to Protected Health Information under HIPAA. Access to Protected Health Information is limited to the extent required to perform responsibilities effectively and efficiently. No access will be permitted unnecessarily. The essential job functions as described, herein, dictate the level of access, use, and disclosure of confidential information
  • Assure all paperwork is collected from ORR upon arrival and admissions
Required Experience
  • Familiarity with OCFS, OPWDD, OMH, and other relevant municipal and state agencies

  • Bilingual in English-Spanish required. All agency documents and records are in English, so English writing proficiency required. Position itself requires Spanish speaking, writing, and reading proficiency
  • Associates degree preferred or High school diploma required
  • Ability to travel to other agency locations as required
  • Experience working in a busy human services office- strongly preferred
  • Ability to handle confidential material in a responsible manner
  • Strong skills in Microsoft Office, Word, Excel and PowerPoint; Strong data entry, documentation and proofreading skills
  • Demonstrated ability to perform duties with minimal supervision and to exercise greater judgment
  • Ability to work well with people from many cultures including recent immigrants, non-English speakers, and LGBTQ children; strong teamwork skills
  • Strong communication and organizational skills, with an ability to handle numerous tasks
Physical Requirements
  • Must be able to stand for 3 hours or more hours
  • Must be able to lift up to 50 pounds
  • Must be able to sit and work on the computer for more than 2 hours
  • Ability to walk up and down stairs repeatedly
  • Ability to lift at least 50 pounds
  • Ability to sit or stand for prolonged periods of time
