Intake Specialist Chha Jobs

Ultimate Care NY, a leading provider of home health care services in New York, is seeking an Intake Specialist to join our team. As an Intake Specialist at Ultimate Care, you will be responsible for coordinating and managing all aspects of client referrals, admissions, and case assignments. You will serve as the initial point of contact for patients and their families, as well as healthcare providers, ensuring that all necessary information is gathered and documented accurately.

Responsibilities

• Primarily handling CHHA & Medicaid needed referrals
• Receive referrals from referral sources
• Communicate with various CHHA’s, Social Worker’s, and marketers to ensure all documents are readily available to assist with enrollment
• Send referrals to appropriate CHHA for skilled services.
• Process patient referrals demographics required basic clinical information and financial reimbursement information.
• Verify patient’s insurance to determine eligibility
• Assisting PT with getting CHHA services

Requirements

• Fluency in English and Spanish (preferred).
• Working knowledge of State, Federal, HIPAA and JACHO guidelines/regulations.
• Knowledge of CHHA and MLTC regulations and requirements
• Excellent interpersonal and communication skills.
• Detail-oriented with strong organizational and problem-solving skills
• Proficient in Microsoft office and Excel applications.
• Bachelor's degree in a healthcare-related field or equivalent work experience
• Minimum of 2 years of experience in a home health agency or hospital setting
• Understanding of communicating effectively with employees, patients and their families, medical and community affiliates to develop positive relationships.
• Experience with electronic health record systems
• Understanding what documents are required for different types of insurance plans
