Job Description

Remote In-Home Services Case Manger - Pediatrics
Saint Luke’s Crittenton Children’s Center in South Kansas City, MO is seeking an In-Home Services Case Manager to join their Pediatric Behavior Health Team. This is a remote position that will be asked to go on site for team meetings and will work with patients in their home. This is a grant funded position.
This position provides In-Home Community Mental Health Services to pediatric behavioral health patients and their families. In this role, you will see 5-8 children and families multiple times each week. Each patient is different and will require unique individualized care.
The Work
  • Coordinate closely with Intensive In-Home Therapist to coordinate the acquisition of basic necessities for families, coordinate with schools, and provide one on one parenting instruction and education to biological and foster families.
  • Must be independent worker with willingness to be flexible with work hours based upon the needs of the families they serve. Utilization of evidence based programming including Attachment, Self Regulation, and Competency (ARC.)
  • Share monthly written progress notes about progress of visits, concerns, and any noted changes with therapist and supervisor on families assigned.
  • Research of area community resources and connecting families based upon their need.
  • This role will provide care to various counties around KC metro
  • This role will share coverage for crisis on-call phone
Must have a Bachelor's in Social Work, Counseling or related field and a clean driving record with applicable vehicle insurance.
