Job Description
What You'll Be Doing
We are currently seeking a Social Worker to become part of our hard working team at the South Wilmington Street Center 24 hour homeless shelter for men. The purpose of the Social Worker position is to provide Intensive Case Management Services and group leadership to the guests that access the South Wilmington Street Center. The focus of this position is to assist individuals needing our services to transition into permanent housing in a timely manner.
Essential functions:
Conducting two groups each week for guests that are enrolled in the Progressive Housing Program (PHP)
Review of each guest’s progress during the past week and extending the guest’s time in the program for another week if progress warrants an extension
Inform guests of training and educational opportunities offered in the community as well those being offered at the SWSC
Document each guest’s progress in NC HMIS on a weekly basis.
Actively assist guests with options to affordable housing in the community to ensure timely movement back into the community
Provide aftercare services to guests that have moved into permanent housing to assist them with any needs and address any problems that may arise to help them maintain that housing
Provide Intensive Case Management Services to a few guests that have difficulties in functioning in a group setting (Between 3 to 7 guests)
Meet daily with the SWSC Case Management Team to discuss daily activities of the guests and report on any guest’s concerns
Meet monthly on the second Wednesday of each month at the All Staff Meeting
Remain current with OSHA, First Aid and CPR trainings

The Basics (Required Education and Experience)
Bachelor's degree in Social Work or related field
One year of job-related experience
Valid Driver's License and "safe" driving record
Equivalent education and experience are accepted
Beyond the Basics (Preferred Education and Experience)
Bachelor's Degree or equivalent
Over 2 years experience
CPR and First Aid certified preferred
