Job Description
Overview


The Hospice Care Transition Coordinator is primarily responsible for facilitating a seamless transition for patients discharging from an acute setting to the home or inpatient facility for hospice services.


Responsibilities


Intake Duties:


  • Utilize critical thinking, reflection, and conscious attention to detail to avoid reactionary behavior
  • Responsible for switching out equipment from non-contracted companies either during intake or admission without compromising patient comfort.
  • Demonstrate strong teamwork, respect, and communication when working with referral partner staff to maintain collaborative relationships.
  • Effectively educate patient/family regarding the philosophy, CFL services, the admission procedure, crisis prevention, and right/responsibilities during the provision of hospice care.
  • Work as a liaison with discharge planners/social workers and physicians at Huntsville Hospital.
  • Task appropriate provider staff to coordinate patient treatment if hospice is not appropriate or desired.
  • Provide a safe and welcoming atmosphere for families dealing with traumatic situations, including death.
  • Participate in system-wide and community-based education utilizing peer-to-peer supportive education skills.
  • Use appropriate criteria (NHPCO, Medicare, Medicaid, CFL, and/or others as required) to evaluate and determine eligibility for admission under the appropriate insurance benefit utilizing an interdisciplinary team approach to evaluation.
  • Follow hospital policy and procedure when coordinating comfort care consults and discharges using multiple electronic communication tools.
  • Responsible for coordination of patient needs in the home such as comfort kit medications, medical supplies, DME, infusion services and/or services identified either during intake or admission.

  • Maintain a log of all tasks and referrals from Huntsville Hospital.
  • Report all process issues, safety concerns, and obstacles to high quality patient care to your supervisor.
  • Responsible for the care and use of assigned cell phone and laptop.


Admissions Duties


  • Responsible for notifying pharmacies of patient’s admission and coordinating pharmacy services.
  • Responsible for taking referrals, requesting permission from the attending physician to admit, and arranging admission time with patients and caregivers.
  • Initiate standing orders, wound orders, etc. to ensure timely delivery of symptom management and care.
  • Responsible for initiating hospice chart in skilled nursing facilities and ensuring all appropriate facility staff are notified of hospice admission.
  • Responsible for accurate and timely completion of admission paperwork and admission chart.
  • Responsible for assuring nursing home patients are in non-skilled beds (including letters if needed).
  • Review patient medications to ensure the most clinically effective and cost effective interventions for patient comfort using the preferred CFL formulary and clinical manager support.
  • Initiate a care plan to achieve patient goals. Task appropriate team members for on-going support.
  • Maintain an accurately inventoried car stock of supplies to support 1-3 days of patient care on admission.
  • Responsible for follow-up when someone is not admitted or declines services.
  • Responsible for on-call duties and all other duties as assigned.


Qualifications


Education: Graduate from an accredited School of Nursing


License, Certification and/or Registration: Current Alabama or multistate Nursing license. BLS for Healthcare Providers required in 30 days.


Experience: 2 year of nursing experience.


Additional Skills/Abilities: Must be able to lift, position, and/or transfer patients. Must have visual acuity and hearing to perform required nursing skills. Duties also require prolonged or considerable walking or standing, reaching, stooping, bending, kneeling, and/or crouching.
