Job Description

Family Hospice Canterbury Place In-patient hospice unit is hiring a Hospice Aide for the team.

Join a collaborative team, who work as an interdisciplinary group to support patients at the end of life. The unit is a 12 private bed unit with support from nursing team and physician. The shifts are 12-hour shifts rotation 7 a.m. to 7:00 p.m. and next week 7:00 am. to 7:00 p.m. Have ability to self-schedule with weekend, nights, and holidays included.

Responsibilities:

  • Communicate appropriately with the interdisciplinary IPU care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
  • Participate in all required agency meetings.
  • Fulfill agency responsibilities by completing all required forms accurately and in a timely manner.
  • Identify, develop and participate in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • One (1) year experience - Preferred.
  • Certified Nurse Aide (CNA) Preferred
  • OAPSA
  • Must have successfully completed a training and competency evaluation program as specified in Home Health and/or Hospice as applicable: Condition of participation: Home Health aide services. Training will be provided by the team.
  • Must maintain annual competency evaluation scores as determined by regulatory requirements. Licensure, Certifications, and Clearances:
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Certified Nursing Assistant (CNA)
  • Act 34
  • Act 33 with renewal
  • High School Diploma or GED - Required
