Job Description
We Care Home Health is a small, growing business in Firestone, CO. We are professional, supportive, rewarding and our goal is build a dynamic team of nurses to care for our private duty and intermittent care clients.

Registered Nurse - Home Health


Duties:


  • Perform comprehensive assessments and develop individualized care plans
  • Provide health coaching and support for patients with chronic conditions
  • Administer medications and treatments as prescribed by physicians
  • Provide skilled nursing care to patients in their homes
  • Educate patients and their families on disease management, medication administration, and self-care techniques
  • Monitor and evaluate patient progress and adjust care plans as needed
  • Coordinate with other healthcare professionals to ensure continuity of care
  • Assist with discharge planning and transition of care from hospital to home
  • Perform specimen collection processing as required


Experience:


  • Familiarity with electronic health records
  • Experience in providing care for patients across the lifespan, including infants, toddlers, and individuals with dementia
  • Minimum of 2 years of nursing experience, preferably in a home health or similar setting

Requirements:


  • Valid Registered Nurse (RN) license in the state of Colorado
  • Ability to lift and move patients as needed
  • Strong communication and interpersonal skills
  • Current CPR certification
