Job Description
Our Mission Home Health team is growing!

We are in need of a Quality Care Manager to review patient assessments and ensure appropriate and effective care is provided.

This role is critical in the development of performance improvement and our education plans!

This role will be remote, but will require minimal travel to our Oregon locations (Portland, Bend, Salem). Pay begins at $80k per year. We offer benefits (medical, dental, vision, life) mileage reimbursement and more!

Essential Job Functions/Responsibilities

  • Monitors performance improvement initiatives regarding patient outcomes
  • Ensures visit utilization is supported by individual patient assessment
  • Collaborates with clinicians as necessary regarding documentation
  • Reviews and approves final Plan of Care (485) within HCHB
  • Maintains working knowledge of home health regulations, conditions of participation and OASIS
  • Reviews OASIS for inconsistencies and locks timely
  • Reviews electronically submitted documentation nd provide feedback to clinicians regarding deficiencies or OASIS corrections to ensure the medical record reflects compliance with skilled need, medical necessity, and homebound status if required
  • Partners with team members to ensure adequate coverage of workflow for each office
  • Attends Case Conference periodically and is available to staff for promoting professional growth and development
  • Partners with the Quality Director and Education Director on a variety of quality initiatives
  • Demonstrates the ability to utilize SHP data to improve quality and optimize performance with real-time actionable analytics
  • Ensures clinical documentation supports Recertification/Discharge decision

Position Qualifications

  • Strong computer and software skills with knowledge.
  • Current unencumbered license to practice as a Registered Nurse in Oregon.
  • Ability to handle stressful situations/deadlines.
  • Must be a graduate of an accredited School of Nursing
  • Minimum of 1 year of home health experience with a working knowledge of Title XX11, Medicare Home Health Conditions of Participation, and OASIS
$80,000 - $90,000 a year
