Job Description

Under the supervision of the registered nurse, utilizes the knowledge and practical experiences of a licensed practical nurse to provide direct patient/family care in the home. Performs basic therapeutic and preventative nursing procedures.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

1. Provides direct patient care to patients assigned by the registered nurse and in accordance with physicians' orders and the established plan of care.

2. Identifies factors affecting health care by evaluating the patient's physical, mental, emotional, environmental and social needs with regard to clinical diagnosis and medical condition.

3. Conducts a complete physical assessment of the patient by obtaining vital signs, wound observation, monitoring lung sounds and reviewing laboratory tests and x-ray studies related to the problem or illness.

4. Provides a variety of semi-complex treatments and/or nursing procedures such as, but not limited to: application of sterile dressings and the cleansing/irrigation and packing of wounds and incisions. Provides ostomy and tracheostomy care; performs Foley, straight and supra pubic catheterizations; performs Foley irrigations, suctioning, pre-filling insulin syringes, urine tests, blood glucose with finger pick blood sample, decubitus care, hot/cold compresses, enemas, fecal impaction removal, collection of lab specimens for urine/blood analysis via direct venous access and other patient treatments.

5. Administers medications and treatments as prescribed by the physician and monitors the patient's response. Notifies appropriate nursing and/or medical personnel of changes in patient's status.

6. Provides basic personal care such as bathing, feeding, backrubs, linen change, ambulation assistance, gait training and range of motion (active or passive).

7. In conjunction with the primary registered nurse, re-evaluates the patient's needs and the effectiveness of services rendered on a regular basis.

8. Reports and records to the primary nurse and physician in charge of the case significant information and changes with regard to the patient's condition.

9. Attends inservices, team meetings and other required meetings. Maintains a safe work environment and practices personal safety techniques to prevent injury to self and others.

10.Promotes and ensures Home Health Care's commitment to the Henry Ford Experience and Team Member Standards of Excellence

11.Complies with corporate and divisional policies and procedures.

12.Maintains accurate and timely clinical records.

13.Responsible for assisting client in learning appropriate self-care techniques, increasing the patient's awareness of medical condition and teaches the prevention of illness and maintenance of general health.

14.Provides counseling to patients and their families in meeting nursing needs.

15.Performs other related duties as required.

EDUCATION/EXPERIENCE REQUIRED:

 Successful completion of a licensed practical nursing program accredited by the National League of Nursing.

 A minimum of two (2) year hands on clinical LPN experience required; prior home health care experience preferred.

 The ability to effectively communicate with patients, families and health care personnel.

 Strong interpersonal and customer service skills required in order to interact effectively with individuals from various cultural and socio-economic backgrounds.

 Must have reliable transportation; possess a current State of Michigan driver's license; and have a satisfactory driving record.

 Must be able to lift, carry, transport and move clients, if needed.

CERTIFICATIONS/LICENSURES REQUIRED:

 Current licensure by the State of Michigan as a licensed practical nurse (LPN).

 NAPNES certification or successful completion of pharmacology exam.

WORKING CONDITIONS:  Works in the patient's home which is an uncontrolled environment.  May be exposed to potentially infectious materials, bloodborne-disease pathogens and hazardous waste.  Drives to and from the patients' homes located in the Detroit Metropolitan area in varied weather and traffic conditions.  May need to lift, carry, bend, stoop or be otherwise physically active in the normal course of job duties.
