Job Description

Organization:

SSC-Sutter Senior Care

Position Overview:

Performs routine patient care, support and related non-professional household services necessary to meet the hygiene, activities of daily living, and comfort needs of the homebound patient, in accordance with the legal scope of practice and within established standards of care, policies, and procedures. Integrates education, technical competence and patient interaction skills to gain confidence and cooperation from the patient, their family/support group, and other healthcare providers. Exercises sound judgment, and developed clinical skills to support the patient's plan of care based on the diagnosis in a timely manner. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety to help deliver optimal patient care. May also be responsible for performing specific procedures and/or teaching duties.

JOB ACCOUNTABILITIES:

NURSING SERVICES
  • Ensures that routine and priority tasks are completed within established departmental time frames.
  • May provide light housekeeping duties or meal preparation in accordance with personal care plan.
  • Provides appropriate, direct patient care to patients under the supervision of the Registered Nurse or therapist.
  • Explains and educates the patient on process of examinations, tests and medical procedures in a clear and professional manner to alleviate fear and anxiety.
  • Performs selected procedures, including but limited to: taking vital signs (temperature, pulse, respirations and blood pressure), collecting specimens, applying bandages, and assisting with prescribed exercises.
  • Maintains equipment and understands procedures and treatments performed by physicians.
  • Mentors and fosters a constructive environment that helps students and new Licensed Vocational Nurses (LVNs) build confidence in their skills, knowledge and abilities.
  • Provides personal care and assistance related to the activities of daily living.

CLERICAL
  • Performs general clerical tasks, such as answering phones; updating patient appointments; or retrieving, entering, and revising patient electronic health records (EHR).
  • Prepares and completes accurate documentation related to patient, including all treatment performed, in medical and departmental records as required.

SAFETY
  • Maintains a clean, neat, and safe home environment for patient.

COMPLIANCE
  • Maintains strictest confidence of all patient protected health information (PHI) and protects all PHI from accidental, intentional, or inappropriate disclosure.
  • Performs quality care in compliance with department standards, policies, and procedures, as well as local, state and federal regulatory agencies to deliver the highest level of patient privacy and care.
  • Retains certification and completes mandated continuing education requirements to meet facility and regulatory agencies accreditation and licensure requirements.

EDUCATION:
Equivalent experience will be accepted in lieu of the required degree or diploma.

HS Diploma or equivalent education/experience

CERTIFICATION & LICENSURE:

HHA-Home Health Aide

BLS-Basic Life Support Healthcare Provider

DEPARTMENT SPECIFIC CERTIFICATION & LICENSURE:

Department: SCAH - California Driver's License

TYPICAL EXPERIENCE:


1 year recent relevant experience

SKILLS AND KNOWLEDGE:

Demonstrated knowledge of medical terminology, healthcare practices and universal precautions.

Ability to interpret a variety of data and instructions, furnished in written, oral, diagram, or schedule form.

Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.

Well-developed time management and organizational skills, including the ability to prioritize assignments and work within standardized operating procedures and scientific methods to achieve objectives and meet deadline

General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook).

Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.

Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.

Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.

Build collaborate relationships with peers and other healthcare providers to achieve departmental and corporate objectives.

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
