Job Description
Overview:
Serves as a health and wellness coach for participants identified as needing wellness coaching services. Using a variety of communication methods including telephonic, in-person, and email, the coach will assist these participants in identifying their readiness to change, and then give support, tools and information to assist them in achieving behavioral, life-style changes supporting good health. The coach works with identified populations, using such tools as behavioral and physical assessment tools, and coaching/educational interventions. The coach will monitor and evaluate for cost effective and quality outcomes.

Qualifications:
  • Computer literacy to include maintaining spreadsheets, charts, and tables of statistics in accordance with quality measures and utilization standards. Multi-cultural experience preferred. Bilingual a plus.
  • Ability to assess clients for readiness for behavioral change.
  • Masters degree preferred.
  • Ability to conduct biometric screening measurements of blood pressure, height, weight, waist and body composition.
  • Ability to conduct behavioral coaching assessments utilizing established tools and criteria.
  • Three years of experience in health and wellness related field.
  • Computer knowledge and skill to include Windows, word processing and database systems.
  • Strong telephonic customer service skills.

Education:

Essential: * Bachelor Degree

Responsibilities:
  • Co-develops an individualized, goal oriented action plan with participant using motivational interviewing techniques of identification, examination and resolution.
  • Performs other functions as required.
  • Conduct health and wellness workshops and educational programs in group settings.
  • Willingness to be trained in and perform blood panel assessments using the finger stick method.
  • Perform biometric screenings on assigned population.
  • Uses the behavioral coaching assessment instruments, screenings and tools.
  • Assesses assigned population for readiness for behavioral change
  • Provides coaching and education to participants on behavioral change.
  • Educates participants about life style choices that support health and wellness.
  • Interpret and relay biometric results and significance of results to participants.
  • Monitor and document specific coaching interventions on a regular basis, using appropriate tools and methodologies.
  • Successfully works within the constraints of role by recognizing when a referral to a different level of care is indicated and providing the appropriate referrals.
  • Willingness to travel within the State of New Mexico with some overnight stays required.
