Description
Health Educator - Cancer Navigator
Remote - within 50 miles of an Elevance office.
Hours: Monday - Friday; 8 am - 9 pm EST
As a Health Educator you will be responsible for responding to inbound inquiries for disease management or medication alert programs. This role will support an innovative "Cancer Navigator" solution. This will be an end-to-end member support program, aimed towards simplifying the complexities of cancer care for our members by connecting them to critical resources and aligning with member care providers.
How You Will Make An Impact
  • Coordinates with other health professionals, provider/s, and external resources to provide feedback on member goal attainment and address clinical, SDOH, and other needs.
  • Implements coaching plan by using behavior change principles to identify member barriers and develop ways to overcome those barriers.
  • Receives or places calls to members who have failed to enroll with disease management to encourage their enrollment in the correct disease management program.
  • Monitors and evaluates the interventions and modifies based on feedback from member and provider/s.
  • Trouble-shoots patient difficulties, which include medications, appointments, services, finances, living and family situations.
  • Compiles and maintains a list of resources to close care gaps and address concerns.
  • Coordinates specific health coaching and support to address needs identified by the member to address objectives and goals identified during cancer and/or other assessments.
  • Conducts behavioral assessments to identify individual member knowledge skills, behavioral, social, financial, and other needs.
  • Coordinates with oncology provider/s to help manage overall cancer care and treatment plan.
  • Diffuses situations with members who are resistant to health coaching and/or disease management interventions.
  • Makes outbound attempts to program members to provide health information according to program standards.
Minimum Requirements
  • Requires BA/BS in appropriate field of specialization and minimum of 2 years of related experience in direct care health education exercise instruction or patient education; or any combination of education and experience which would provide an equivalent background.
Preferred Skills, Capabilities And Experiences
  • Certification or advanced certification and/or license appropriate to field of specialty preferred.
  • Prior experience in health coaching, disease management in a call center environment, and/or patient education and behavior change techniques and strong understanding of disease management principles is preferred.
  • For candidates working in person or remotely in the below locations, the salary* range for this specific position is $24.12 to $43.40 **
  • Multi-state licensure preferred in accordance with company policy.
  • Experience working with Oncology and Hospice Support.
Locations : California, Colorado, Nevada, New Jersey (Jersey City only), New York (New York City, Ithaca, and Westchester County only), and Washington State
