Job Description

Mission:

The mission of Bread for the City is to help Washington, DC residents living with low income to develop the power to determine the future of their own communities. Position Profile:

Provide support for clinical and health education services, as well as perform a variety of administrative activities to support the functioning of the clinic. In addition, the Community Health Nurse will support the agency in risk management activities to support a safe environment for clients and staff. As a member of the Bread for the City team they are expected to be familiar with the philosophy, goals and objectives of Bread and work collaboratively to achieve them.


Essential Responsibilities:

Clinical – provides support during clinic hours and assists patients with more complicated medical problems to navigate the healthcare system.

  • Works with providers to help with medical case management for patients with more complicated medical problems;
  • Serves as vaccine coordinator in SE clinic, overseeing inventory, storage/handling, ordering, and ensuring clinic is compliant with DC Department of Health vaccination programs;
  • Assists with lab, including performing phlebotomy, injections, EKGs, specimen processing and handling
  • Assists with follow-up of patients after Emergency Department visits and hospital discharges.


Risk Management/Administrative - guides the risk management plan and program; analyzes event reports; responds to patient grievances, complaints, and claims; educates staff on risk management and safety; and communicates with individuals at all levels of the organization.

  • Supports the Chief Medical Officer in oversight and coordination of the health center’s risk management activities;
  • Identifies areas of risk and develops additional policies and procedures to address them as necessary;
  • Leads Emergency Preparedness Team, ensuring compliance with all internal policies and local and federal regulations;
  • Monitors compliance with all internal policies and local and federal regulations related to the Clinic;
  • Serves on the Quality & Risk Management Committee; manages, collects and evaluates data concerning patient incident reports and develops monthly and quarterly risk management reports that include trend analyses of incidents and complaints.
  • Works with nurse to manage medical supply and medication inventory and ordering
  • Represent the organization in a variety of coalitions to support activities of public health such as the DCPCA Emergency Preparedness Working Group and DC Health Matters Collaborative

Health Education

  • Works with the provider and social services staff to provide health teaching for prevention and management of patients’ medical conditions;
  • Provides clinical support to front desk staff and the health resource room.


Qualifications:

  • Strong desire to work with our patient population;
  • Licensed RN in Washington, DC;
  • Candidates that speak other languages such as Spanish, Amharic, or Mandarin are preferred;
  • Ability to work with volunteers and staff in a professional and collaborative manner;
  • Familiarity with Google Organization & Productivity Tools (Gmail, Google Docs, Calendar) and/or ability to learn new technology systems;
  • Willingness to learn and carry out Bread for the City's commitments to social justice and racial equity.
