Geriatric Social Worker
The Geriatric Social Worker will join our team of professionals to help meet the needs of veteran residents in our Veterans Support to Self-Reliance (VSSR) Pilot program. This position is a wonderful opportunity for the right candidate to utilize and grow their social work and management skills to serve our participants and oversee our vital intake process. This position reports to the Program Coordinator and works under the supervision of the Director of Behavioral Health and as a member of a multi- disciplinary team. The Social Worker is responsible for the assessment and reassessment of participants in the VSSR Pilot program.
FLSA Classification: Non-Exempt
Responsibilities:
  • Completes internal and external reporting for grant funders to include but not limited to the funders that guide the work of assigned staff and other internal reporting needs as assigned.
  • Provide training to case management and property management staff about aging-related health declines and behavioral shifts that may impact housing retention.
  • Coordinate with transportation and peer specialist staff to transport and accompany residents in securing benefits and assistance.
  • Assist veterans to apply for and manage benefits.
  • Connect veterans with appropriate senior resources.
  • Develop appropriate care plan for individuals admitted to the program, monitor implementation, document progress of care plan and revise as necessary, on a bi-annual basis at minimum.
  • Provide counseling, support and intervention to participants and caregivers in the areas of behavioral management, caregiver stress, coping strategies, and other issues as needed.
  • Meet with Pilot staff to identify residents needs such as accommodations for disability, income assistance and other needs.
  • Lead medication management support.
  • Report suspected verbal, physical, fiduciary, and/or sexual abuse to appropriate monitoring agencies.
  • Other duties as assigned.
  • Provide transportation using company vehicles for offsite activities as needed.
  • Assist veterans in making medical appointments, coordinating transportation and accompanying veterans as needed.
  • Help veterans connect with and engage in tele-health services.
  • Meet with Pilot staff regularly to assist them in resident interactions and trauma informed communications and service delivery.
  • Serve as a liaison with the participants caregiver and other community services involved in care and with the inter-disciplinary team as needed.
  • Monitor participant attendance and case mix and center daily attendance.
  • Facilitate weekly and or monthly support groups when appropriate.
  • Assist veterans to utilize community resources and leverage engagement opportunities.
  • Discuss long-term care plan and schedule a discharge meeting with families and Program Director.

  • Driver history report which meets company insurance requirements to drive company vehicle and complete a provided driver training course upon hire.
  • Must be flexible, positive and cooperative in solving problems and working with other staff in constantly changing and sometimes stressful circumstances.
  • Must be able to move around the facility as well as sit at a workstation. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Qualifications
    • Experience working with populations that are diverse in age, gender, sexual orientation, race, and ethnicity.
    • Experience working with trauma survivors and those experiencing mental illness.
    • Sensitivity in working with patients with complex psychosocial and substance abuse problems.
    • Experience doing case management, outreach, and home visits.
    • Able to move between different treatment approaches (clinical case management versus evidence-based, trauma-focused psychotherapy) depending on the needs of the client.
    • Must be able to communicate effectively with a variety of personalities and be comfortable working with an at-risk client population, including embracing the agencys DEI Initiative.
    • Valid California drivers license and auto insurance required. Travel between all housing sites is required (mileage reimbursed)

    • Current enrollment in the final semester of a Masters program at an accredited college or university leading to a Masters Degree in social work or a Masters Degree that meets the education requirements for a Marriage and Family Therapist or Licensed Professional Clinical Counselor license issued by the California Department of Consumer Affairs Board of Behavioral Science Examiners.

  • Must be able to carry out job instructions promptly and willingly, and to follow established procedures accurately.
  • Experience in counseling, facilitating support groups and conducting participant assessments is preferred.
