Job Description
  • JOB REQUIREMENTS

  • Experience
  • Normal hospital environment. Occasional exposure to fumes, odors, biological and electrical hazards. Normal/corrected eyesight with close eye work. Hearing of normal and soft tones. Uses computer, telephone, copier, fax, scanner and adding machine. Lifting up to 50 lbs., carrying up to 20 lbs., pushing/pulling up to 100 lbs.
  • Must be licensed as a registered nurse in the state of Arkansas.

  • Minimum of one year experience in hospital practice preferred.
  • JOB SUMMARY
  • The Discharge Specialist is responsible for completing the discharge process and insuring the patient is adequately educated. The position must possess excellent communication skills and function at a high level as a member of the collaborative team. The role of this position is pivotal in coordinating the care and education of the patient pre and post discharge to facilitate an efficient and effective acute care stay, and coordinate accurate information to the next provider(s) of care to minimize readmissions. This position is required to utilize independent judgment.
