Director, Population Health Jobs

Job Summary

The director will provide leadership for the strategic value based care/population health management initiatives within the organization. This position will work collaboratively with the Director of Practice Management and Director of Quality to achieve all goals set by the organization and the accountable care organization (ACO).

This role will be responsible for working with physician leadership to develop and implement value-based care/population health management processes, workflows and achieve metric goals.

Minimum Requirements

Bachelor’s degree in business or health administration or equivalent degree. A minimum of five years-experience in setting up and managing value-based care/population health management initiatives in an outpatient setting. Commercial payor contracting experience, especially with risk based contracting. Compensation and incentive alignment expertise and background for value based care/population health management. Use of data analytics to improve care and performance. Skilled in team building and communication with directors, physicians/APPs, staff and others.
